The following is an overview and weighting for the rated criteria of the RFP 17014 - Domestic Water and Sanitary Piping Replacement at 171 Front Street. Respondents are asked to clearly demonstrate in their submission that they are capable of undertaking this assignment.
- Company Description and Experience(15 points)
1.1.Proposals must provide the overview of your company. It will be evaluated based on the following requirements: (3 points)
-type of business (related to the project);
-company’snumber of years of experience in plumbing and sanitary services;
-volume of work (dollar value) in plumbing and sanitary for last three (3) years;
-experience in working in residentialbuildings (e.g. single family, town houses, multi-unit residential buildings)
-capacity to implement projects with the value over $500,000
1.2.Provideas an attachment anorganizational chart - a project specific chart highlighting (3 points):
-entire team by position/titles/roles
-number of employees for each position, including any support personnel
-key personnel who will be assigned to the project as identified in the scope of work
1.3.Proponents must provide resume for three (3) key personnel; which must include the following information (maximum of 9 points – 3 points per each resume):
-years of experience in the area
-relevant education
-professional certificates related to the area (mechanical construction: plumbing and sanitary services) – copies of certificates must be submitted
-trainings related to the field (mechanical construction: plumbing and sanitary services)
-dollar value of managed work in the category
- Health and Safety Processes (10 points)
2.1. Workplace Injury Summary Report (WSIR) for 2014, 2015 and 2016 (4 points)
WISR should demonstrate the record of injuries over three years. Proponents with 0 number of injuries will get maximum points, half of the points - for up to 7 injuries, and zero points - for more than 7 injuries over the last three years.
2.2. Please provide site specific measures that will be implemented to ensure safety during the construction period for (3 points):
-construction team
-building tenants
2.3. Please provide threesite specific measures if multiple contractors will be working on separate projects in a building. (3 points – 1 point per site specific measure)
- Scheduling (10 points)
Submissions will be evaluated based on contractors understanding of the project, details provided in the schedule, start and end dates, and timely completion.
3.1. Please provide as an attachment proposed detailed construction schedule in Gantt chart. Gantt chart must include (3 points):
-start date
-end date
-substantial completion
-final close out meetings
-commissioning
3.2. Please describe your approach to scheduling, schedule tracking and progress reports. (3 points)
3.3. Based on experience with similar projects, describe your approach to maximizing efficiency in work for this specific project. (4 points)
- Communication (10 points)
Submissions will be evaluated based on the communication procedures and responsiveness to correct problems once identified, as following:
4.1. Please provide overview of your internal structure as it pertains to service escalation (tracking communication, escalation and resolution). Outline problem resolution process to determine root cause issues and measures to ensure the completion of resolution. (5 points)
4.2. Please provide as an attachment the Issue log (problem and response time). (5 points)
- Deficiency and Quality Control (10 points)
Submissions will be evaluated based the approaches used by the Proponent to manage the quality & deficiencies.
5.1. Provide three examples of the tools that you would use in the execution of the current project to control the quality & deficiencies. (6 points – 2 points per each example)
5.2. Describe your process for managing the correction of deficiencies, in particular (4 points):
-Internal structure
-Internal processes
-Internal and external review
-Monitoring
-Reporting
- Performance History (References) (total 45points - 15 points each)
Proponents are required to provide three (3) project references. Projects provided must meet the following criteria:
-company was a principal contractor
-minimum project dollar value of $500,000
-substantially completed in the last five (5) years
-related to the scope of work identified in this RFP (plumbing and sanitary)
-implemented in residential buildings (e.g. single family, town houses, multi-unit residential buildings)
TCHC may contact proponent’s references provided in this rated criteria. The purpose of the reference check is to validate the information provided by the proponents, regarding the quality of work, adherence to schedule, adherence to budget, ease of resolving disputes, response regarding change orders. TCHC reserves the right to disqualify any proponent that provides false or inaccurate information.
