SUBJECT: Notice for the submission of bids in an international closed tender

The Greek Atomic Energy Commission calls for a closed tender, the awarding criterion for which being the underbidding price, for the Procurement of a Co-60 sealed radioactive source (148TBq-4000 Ci), its in situ installation on radiator Head Model C-9-R Teletherapy Head AMS/PICKER HEAD s/n209, which is currently installed at the Laboratory of Ionizing Radiation Instrument Calibration of the Greek Atomic Energy Commission, and the dismantling, transportation and exportation of the existing used radioactive source AMERSHAM TYPE X.4016 CAPSULE, s/n 0558ET with radioactivity 148.5 TBq, 4000 Ci on 29/03/1999. The budget for the afore-mentioned procurement is foreseen to run into the net amount of 100,000.00 euros, CIF Athens, exclusive of VAT and bank costs. This amount may be surcharged with customs clearance costs when the supplied equipment originates from any non EU country, plus any other cost that may be required for the completion of such procurement.
Tenderers shall submit their printed participation application in duplicate, before the Greek Atomic Energy Commission, Financial & Procurement Department, Patriarxou Grigoriou & Neapoleos, FAX. +30-210-650 6711, +30-210-650 6748, tel. +30-210-6506806.
Applications must include: Full name, registered office, postal address, fax and telephone numbers, business activity.
The documents to be submitted by tenderers along with their Participation Application are as follows:
1. Operation license for works with high activity in radiation sources (installation, dismantling, transportation etc) issued by the competent authority of the country of establishment.
2. An ISO 9000 certificate for the manufacture and/or sale and installation of radiation sources used in radiotherapy.
3. A solemn statement Νο.Ι for Tenderers’ personal status, according to Greek Law 1599/1986, or any other equivalent document for non resident candidates, legally certified for signature authenticity, where Tenderers shall declare that, until the bid submission date, each Tenderer:
1. is not bankrupt, wound up or under any other analogous situation (save for legal persons operating under public law, local government organizations and public organizations);
2. is not the subject of proceedings for a declaration of bankruptcy, winding up or any other analogous situation;
3. has fulfilled his obligations relating to the payment of social security contributions, as of the Tender date;
4. has fulfilled his obligations relating to the payment of taxes;
5. has not participated in any criminal organization, as defined in article 2(1) of Council Joint Action 98/773/JHA;
6. has not been convicted for bribery, as defined in article 3 of Council Act of 26 May 1997 and article 3(1) of Council Joint Action 98/742/JHA3;
7. has not been accused of fraud, in the sense of article 1 of the Convention on the protection of the European Communities' financial interests;
8. has not been accused of money laundering, as defined in article 1 of Council Directive 91/308/EEC of 10 June 1991, on the prevention of the use of the financial system for the purpose of money laundering;
9.has not been accused of misappropriation, swindling, extortion, forgery, perjury, bribery and fraudulent bankruptcy, acc. to article 8(2) of Presidential Decree No. 118/07.
10. is not guilty of having submitted false or incomplete statements in submitting the information required hereby;
11.has not been disqualified from any tender participation.
4. Bank statements verifying Tenderers’ creditworthiness (save for public entities).
5. Financial statements, or extracts thereof, or tax assessment statements – for natural persons or for legal persons which are not liable to financial statements – for the last three (3) fiscal periods. In case Tenderers are activated for less than three (3) fiscal years, then they should submit such documentation for as many periods as they are active.
6. A solemn statement on the total turnover of the company, and the turnover involving specifically the equipment under procurement hereby, which must be over 80% of the contracted budget auctioned hereby, for the last three (3) fiscal periods. In case Tenderers are activated for less than three (3) fiscal years, then the accumulative turnover involving specifically the equipment under procurement hereby, for the fiscal years that they are active must be over 80% of the contracted budget auctioned hereby.
7. A solemn statement by the tenderer – manufacturer of the plant where the offered equipment shall be constructed and the place of its establishment.
8. When the Tenderer is not the manufacturer of the final product himself in his own plant, a solemn statement of the plant where the offered product shall be manufactured and its place of establishment, plus a solemn statement that the construction of the final product shall be carried out by the company which owns or manages, wholly or partly, the plant where the final product shall be manufactured, and that the legal representative of such company has accepted against them, the execution of the specific procurement, in case of contract award to the supplier on behalf of whom such acceptance has been effected.
9. Description of the tendering company’s technical equipment and the quality assurance measures taken by the supplier.
10. A list of the technical staff of the tendering company who shall be engaged with the production of the offered product, sorted by specialty, regardless of their contractual relation with the company, especially for quality control management.
11. A list of contracts, including major deliveries of the last three years, indicating for each delivery, its receiver, either within the private or the public sector, its delivery date and its price.
Participation applications to be submitted before this office, inside a sealed envelope, by Wednesday 24/03/2010, at 14.00 hours.
Application submission raises absolutely no commitment for the office calling for this Τender.
