2018-2019 SCHOLARSHIPAPPLICATION

Application deadline: received by 5 p.m. Friday, April 20, 2018

Scholarship Award Amount: One (1) Scholarship will be awarded at $2,000.
One current ASU student will be selected to receive the $2000 award.

Students must reapply each year to be considered for the award.

To be eligible for the scholarship, an applicant must:

  1. Be a current ASU student, enrolled at ASU for the fall 2018 semester for a minimum of 12 hours.
  2. Have a minimum 3.25 GPA
  3. Have residency within the geographic area of Arizona White Mountains (Heber/Overgaard, Pinetop-Lakeside, Snowflake/Taylor, St. Johns, Springerville, Show Low, Whiteriver)
  4. Not participate in Intercollegiate Athletics as an ASU student athlete while attending ASU
  5. May not be a recipient of The Johnson Family Foundation Scholarship.
  6. The award may not be deferred for illness or religious reasons. The posting of award funds each semester will be conditional based on the receipt of current semester transcripts by post or e-mail to the scholarship committee.

Note regarding eligibility:
*Applications will not be accepted/considered for students participating in or receiving scholarships with ASU Intercollegiate Athletics as an ASU student athlete while attending ASU, or their immediate family members.
**To ensure this award’s maximum benefit is received, students who are granted more comprehensive awards may not be granted these funds and a new recipient may be selected.

Application Procedure

Please submit the following in this order:

All documents should be type-written, with the applicant’s name appearing on each document.

Please submit 5 copies of all documents provided in addition to the original.

  1. Completed scholarship application
  2. A type-written statement (maximum 1 page, double-spaced) describing personal & family background, goals for higher education and career goals
  3. Two letters of recommendation, on letterhead, from persons who can attest to potential academic success and character of applicant
  4. A copy of ASU transcript, and include a current class schedule

Among other rights afforded me under Federal Legislation, commonly known as “The 1974 Family Educational Rights Act,” as amended, I understand that I have the right to specify the name of each and every individual to whom my records are released.

I further understand that I may waive my right to specify the name of each scholarship donor or scholarship committee to whom my records are released as long as such records are used only in connection with the ASU Alumni Association. I hereby authorize the ASU Alumni Association to gather and release all records maintained by the office concerning me. Such records may contain the following:

  1. A copy of my unofficial transcript.
  2. Other documents and information relating to my academic performance.

Application Review & Selection Timeline:
The Scholarship Committee will review all completed applications and select 1student to receive the scholarship. The criteria the committee considers per application include the personal statement, activities & honors, letters of recommendation, and overall evaluator judgment. Applicants will be notified via e-mail or phoneafter our June 2, 2018 fundraising event. Please ensure your current e-mail and phone number are included in your application.
