INSTRUCTIONS:

1.  Complete application for approval on all events and/or room requests. A fundraiser form must also be completed if your event includes a fundraiser – Application for events, room requests, and/or fundraisers are due two weeks before the event.

2.  Fill in all appropriate sections (Sound Coordinator, Plant Manager, Athletic Director, specific campus, etc.)

3.  Place this application in Mr. Estoesta’s mailbox at Flagship campus or Ms. Carcamo’s at Olguin campus.

4.  Faculty Sponsors must be present throughout the event. Students are not to be left unsupervised at any time. Students may not be pulled our or kept out of another teacher’s classroom without the written consent of that teacher. There are no exceptions.

5.  If the ensuing requirements are met, event will be calendared/approved and a white copy of this form will be placed in faculty sponsor’s mailbox as confirmation;

*Reminder: Each Club/Sport/Activity will give 50% of all profits to the Student Body Account. 50% of concession sale profits, depending on event prior approval, will be turned in to the Student Body Account. All concessions must be approved by ASB.

All after school and weekend fundraisers for clubs/sports may keep 100% of their profits but forms must still be submitted for approval and to ASB.

**One form per fundraiser. If you plan on conducting more than one fundraiser you must fill out a second form.

***Financial Statements MUST be submitted to Student Body within 2 business days after the event.

You must receive formal notification of approval before selling items. Leadership will notify the sponsor of eligibility or ineligibility four days prior to fundraiser date.
