STEPS IN THE HEALTH CARE WAIVER PROCESS

  1. Meet and interview client.
  2. Draft the application using the samples provided by CGLA.
  3. Gather necessary court documents and supporting documents from clients.
  4. Certified dispositions for all Cook County criminal cases are available from the Clerk of the Circuit Court at the Daley Center in room 1006. Certified dispositions cost $9 each and can be obtained at the time of request. All that is necessary to obtain a certified disposition are the case number and client's name.
  5. In some cases, it may be helpful or necessary to view documents from the criminal case file. Documents from the criminal case file include things like arrest reports, charging documents, and probation information. Obtaining documents from the case file will not be necessary for everyone, but can be useful for building your client's story or to clarify sentencing information and other questions. Felony case files are available at the 5th floor of the Cook County Criminal Court Building located at 2650 South California. To obtain these files, you can call the Criminal Court Building Vault in advance at 773-674-3152, and place a request for the file by providing the clerk with the case number of the file you wish to obtain. Misdemeanor case files are available at the Daley Center, again in room 1006. To order misdemeanor files you can call 312-603-2288. Both felony and misdemeanor files will be ready for viewing 3 business days after the order is placed. After the file is available at the Clerk's office, you can view the file and request copies of any necessary documents from the file. The cost for making copies from criminal case files is $2.00 for the first page, $.50 for the next 19 pages, and $.25 for each page after that.
  6. After meeting with client, drafting the application, and gathering all supporting documents, please email CGLA your draft for review. Please email it in Word format to Courtney Kelledes at . CGLA works to have comments and suggestions on the drafts back within one week.
  7. After the application is finalized, mail the typed narrative, fillable PDF application (must be signed by client), supporting documents, and cover letter to:

  1. Approximately 3-4 weeks after the Registry receives the application, the client will have a request for Livescan fingerprints mailed to the address listed on the application. Clients must have their fingerprints taken at an approved agency (see fingerprint locations). The fingerprint results will be forwarded directly to the registry.
  2. At that point, the Registry will reflect the Waiver application is “Pending.” See
  3. Approximately 3-6 weeks after receipt of the fingerprint results, the client will receive a decision by the Department of Public Health. It will be mailed to the address listed on the application and will be reflected on the website above.
  4. Please email ny documentation from the Department of Public Health that your client shares with you.
