SPECIAL CONDITIONS:
a) Payments will be done via bank transfer – ensure that you supply correct details
b) Drivers must conduct themselves courteously at all times towards personnel or users
c) The drivers will maintain clean and neat physical appearances
d) The drivers maintain all safety and security rules at all the points entered or exited
e) On all mining or port or factory or owners premises the following must be maintained
1- Wearing of hard hats
2- Wearing of reflective jackets
3- Wearing of protective goggles
4- Wearing of overalls
5- Wearing of ear plugs
6- Wearing of safety boots
7- Truck equipped with fire extinguisher 45kgs
8- Truck equipped with orange rotating light
9- Truck equipped with reverse horn
10- Truck equipped with reflective triangles
NB: THE MINE IS STRICT WITH REGARDS TO THE ABOVE REQUIREMENTS. IT IS IMPORTANT TO COMPLY WITH THE REQUIREMENTS OR THE VEHICLES OR DRIVERS WILL BE ESCORTED OFF THE PREMISES

Documents required to enable booking of the vehicles and obtaining the contract:
Completed spreadsheet, Git, Truck and trailer registration papers, Drivers identity docs and licences
All other paperwork will be required after contract signing.
Upon signing this agreement, you the transporter agrees to supply the number of vehicles being ______34 ton side tippers. And agree to the terms laid on this agreement.
