INSTRUCTIONS TO FILL OUT THE REPORT REQUEST FORM AND PRINT THE REQUIRED SCREENS
1. Fill out all necessary data in the first two boxes on the first page of this document. [The requester of the report will sign the last box when the Report is ready to go into live production.] All data will be required to complete the Report. Some most frequently asked questions when filling out the data are as follows:
Ø Do I need the Department/Cost Center data? You would need this data if it were applicable to the report. For instance, if you were requesting a range of departments to be on the report, then the department numbers would be necessary.
Ø What if there are no drop-down menus necessary? If that is the case, just type in N/A for Not Applicable.
Ø What if there are no Calculations, etc. necessary? Type in N/A for Not Applicable
Ø What if I do not have enough room to fill in all my necessary data? There are lines on page three for you to use for added information to create your Report. Page three will also be used for retesting, if necessary.
Ø What is meant by Date Elements [Report Field]? These are all the fields on the screens necessary for the report. [i.e. a name field, a position field, or a payroll date field].
Ø When I receive the Report from IS, do I need to test it? Yes, you will need to make sure the report has gathered the data you expected.
Ø Do I need to test the data when it is put into production? It is highly suggested to test the report in live production to make sure all data converted correctly from test to live.
2. Attach screen prints of the Meditech screens that are involved with your reports. [If a field is necessary to be on your report, then you will need a screen print of the screen where that field resides in Meditech]. These screen prints are necessary so the Report writers will have the detail to produce your reports correctly. To get a copy of a screen print do the following:
Ø Select the screen you want to print in Meditech
Ø When the screen is on the PC, hit the following two keys: Ctrl and Print Scrn, both at the same time
Ø Go into Word on your PC and pull up a blank document
Ø Hit the following two keys: Shift and Insert, when you are in the blank document, hitting both keys at the same time
Ø The screen print will appear in the blank document
Ø Name the document you just printed, the same name as the screen. [The name of the screen will be at the top of the screen in the word document.]
Ø If you have Outlook email, attach this document and the screen print documents to an email message by clicking on the insert button on the toolbar [resembles a paper clip]. Select the document and the screen prints, one at a time, and they will appear in the email. Send the email to the Information System’s employee who is creating the Reports.
Ø If you do not have Outlook email, print this document and the screen print documents and send them to the Information System persons who are creating the Reports.
Ø When IS has completed the report and you have completed your testing; fill out the fourth box on the first page to request the report be put into live production.
Ø Again, keep copies of all documentation in your Word documents on you PC.
