SSVF Housing Case Manager

ESSENTIAL DUTIES, RESPONSIBILITIES AND DESIRED CHARACTERISTICS
- Stewardship
- Contribute to reports for SSVF, as requested.
- Maintain a 90% score on internal clinical and administrative reviews.
Information Management:
- Maintain a file on each participant that includes eligibility documents, strengths assessment, personal goal plan, case notes documenting action taken, progress, challenges, and follow up, and all required agency documents (e.g., Bill of Rights, Release of Information).
- Maintain comprehensive statistical data on all participants.
- Maintain accurate records of financial assistance provided to participants and progress with goals in MAACLink and in internal systems of outcome tracking.
- Prepare case records for proper storage after participant exits program.
- Complete other reports as requested.
- Teamwork and Collaboration
- Develop and maintain a positive supportive relationship with grantor.
- Attend agency and community meetings as requested.
- Attend Veteran Services Team meetings.
- Maintain working relationships with community agencies (e.g., VA, MAAC and Homeless Services Coalition) to provide comprehensive service for participants and to stay abreast of current trends and resources available.
- Attend in-service training and outside conferences/workshops.
- Represent the agency’s mission, philosophy, and function to the community.

- Provide ideas for programming specific to educational or process groups needed for participants.
- Assess employment needs, make referral to Employment Specialist and monitor employment success.
- Assess eligibility for veteran and mainstream benefits; make referral to community partners, as needed, to secure eligible benefits
- Specific performance targets for the 2014-15 VA SSVF contract:
- Serve 25 Veteran families that are homeless;
- Serve 15 Veteran families at-risk of homelessness
Strengths-Based Case Management
- Conduct interviews with potential participants emphasizing their responsibility for participation in the strengths based model of case management.
- Complete personal goal plan with all clients.
- Schedule at least two one hour sessions per month with participant to develop and monitor progress with goals.
- Provide information and referral services.
- Provide advocacy services as needed (e.g., legal, landlord/tenant conflict resolution).
- Provide financial assistance in accordance with program policy and procedures.
- Coordinate case management efforts with all staff and contracted services to meet family and children’s needs.
- Develop plan for ongoing goal setting upon completion of program.
- Maintain contact with participants after completion for the purpose of follow up, outcome tracking and follow up services.
- Perform other duties as assigned by the program manager or director.
MINIMUM EDUCATION and/or EXPERIENCE
Bachelor’s degree in human services required (preferably Bachelor of Social Work) with three to five years case management experience or Master’s degree in human services preferred (Master of Social Work) with one to two years case management experience.
Must possess the following characteristics: great communication (verbal and written); problem solving and conflict resolution skills; knowledge of computers and software programs; ability to work with diverse populations; conversant in and supportive of Catholic Charities mission; possess great time management skills and have the ability to handle multiple demands and priorities. Must have the ability to interact positively with grantee, referral sources, other agencies, and community partners. Must also have the ability to comprehend, document, and converse in Standard English and demonstrated great sound judgment and reasoning skills.
