Spring 2014 Athletic Requirements Letter

Dear New Student,

NCAA regulations require all varsity student athletes to undergo a Physical Exam by a physician within the six months prior to participation. In light of this regulation for pre-participation medical exams, Bates College requires a Physical Exam by a physician each year for students wishing to play a varsity sport. This requirement does not affect most club sports. Those students interested in playing Ice Hockey or Rugby participants will be required to have a Complete Physical Exam by a physician prior to participation.

The following required Sports Medicine forms can be downloaded at or at and submitted to the Bates Health Center:

*Bates College Physical Exam 2014- 2015 Form – must be within 6 months of participation set by NCAA. Due no later than, 1 July.

*HIPAA Form – Bates required no later than, 1 August.

*Emergency Information Form – Bates required no later than, 1 August.

*Concussion/Injury Agreement Form – Bates required no later than, 1 August.

*Sickle Cell Trait – NCAA required for all varsity athletes no later than, 1 August.

Lastly, the NCAA also requires student- athletes who are being medicated for ADD/ADHD toprovide documentation of ADD and ADHD medication(s) from their prescribing physician on the form downloaded from the address above. This information must be submitted by 1 August along with the above forms.

We strongly encourage you to get your physical after 15 June.

Please do not leave any blank spaces on any form and sign the Sports Medicine release on your Athletic Physical Form before returning it to the Health Center.

The Health Center cannot do any pre-participation athletic physical exams. For local options for physicals please go to Local off-campus options for physicals are independent of Bates College and are subject to availability which may delay your participation date. If you arrive on campus without this exam, when you are scheduled to begin practice, you will not be allowed to participate until medically cleared and in compliance with the information needed for clearance. Please plan accordingly and begin scheduling your appointment now. Completed physical exam forms which arrive after the stated deadline will be placed in a “late file”. The “late file” will not be reviewed immediately and will result in a loss of participation time.
