This document confirms the agreement of the following company/institution tobecome sponsor and/or exhibitorof the 8thInternational 3DBODY.TECH Conference and Expo,to be held from 11to 12October2017,in Montreal, Quebec, Canada.
Note: the different sponsor/exhibitor levels are shortly described at the end of this document.
Please fill this form, save it and send it by e-mail to the conference office (see below).
You will then receive an invoice with details for the payment of the total fees.

The prices are indicated in Swiss Francs (CHF) and are inclusive oftaxes when/if applicable.
Payment Terms
Payment methods: bank wire transfer on CHF/EUR/USDSwiss bank account;credit cards (VISA, MasterCard, AMEX) by Saferpay secure e-payment system; PayPal payment system (+6% fees).
Invoicing and payments are possible in CHF, EUR,USD (AMEX credit card: CHF only).
The current exchange rates will be applied, for invoices in USD and EUR.

Bank transfer with IBAN & SWIFT/BIC on USD/EUR/CHF Swiss bank account
Credit card (VISA, MasterCard, AMEX) by secure e-payment, +6% fees
PayPal and other credit cards through PayPal payment system, +6% fees

Description and Logo
Please send us by email a short description of your company/institution anda high resolution image (jpeg preferred) of your logo, which we can add to the conference websiteand use for the conference calls and program.

LEVELS AND BENEFITS FOR SPONSORS/EXHIBITORS
The following levels for sponsors/exhibitors are available:
GOLD SPONSOR/EXHIBITOR
Costs:
2750 CHF basic fees, with 9m2 (100ft2) exhibition space, 2 full and 1 staff registration included;
550 CHF for each additional full registration;
100 CHF for each additional staff at the exhibition;
150 CHF for each additional 1m2 (10ft2) exhibition space.
Limited places of this sponsor/exhibitor level are available (FCFS policy).
The following benefits are offered:
- 9m2=3x3m (100ft2=10x10ft) space at the technical exhibition during 2 full days;
- larger exhibition space possible (150 CHF per additional m2);
- 2 full conference registrations and 1 staff registration included;
- additional conference registrations at discounted fee (550 CHF instead of 620 CHF);
- logo, link and short description on the conference website;
- logo and link on conference calls (call for papers, call for registrations, preliminary program);
- logo, link and short description on conference program.
