Upper Hunter Homeless Specialist and Outreach Service will deliver homeless services in throughout the Upper Hunter. UHH-SOS will deliver an integrated assertive outreach and emergency/transition accommodation service in the Singleton, Muswellbrook and Upper Hunter LGAs. UHH-SOS will work with young people, men, women and families at imminent risk of, or experiencing, homelessness in either crisis or chronic circumstances.

Grade:Social, Community, Home Care & Disability Services Industry Award 2010 – (SCHADS) – Level 4. We also offer some above award conditions.
Hours of Work:38 hours per week

This position provides support, advocacy and information to assist young people, men, women and families working within a case management framework, and appropriate to the individual needs of the client.
Work in this position requires a sound level of knowledge and skills to achieve results in line with the organisation’s goals. This position usually works under established guidelines and procedures.

Key Result Areas
- Work with clients at imminent risk of homelessness to plan, implement and review individualized case plans, including explicit details about goals, mutual responsibilities and services.
- To provide information, advice, referral and advocacy for clients seeking assistance through UHHSOS.
- To monitor clients in crisis to progress in relation to transitional goals and to facilitate seamless exit from service.
- Maintain records and contribute information and advice to support UHHSOS reports and submissions.
- Keep up to date and current on information, resources and research in the area of men, women, youth and family services
- Contribute to community education and awareness programs
- Maintain and provide clear written communication, keeping timely and accurate case notes, letters of support and documented communication with other services pertaining to the clients
Duties
- Participate in intake and assessment roster at the service UHHSOS Hub.
- Facilitate access to specialist service providers for clients at imminent risk of homelessness
- Develop & monitor transition plans for rehoused clients.
- Develop case plans for people experiencing homelessness within 48 hours of assessment.
- Collaborate with other services to coordinate tailored case mix to help newly homeless people to return home.
- Provide robust assessment of clients to determine need for crisis or temporary accommodation.
- Actively provide case management to move clients to longer term housing support.
- Support clients in crisis to access appropriate mainstream services.
- Monitor clients to progress in relation to transitional goals and to facilitate seamless exit from service.
- Assist with development of innovative solutions to housing crisis in the Upper Hunter
- Provide advice to inform development of UHHSOS promotional strategy.
- Actively participate in professional supervision.
- Maintain records and contribute information and advice to support UHHSOS reports and submissions.
- Contribute to development and maintenance of a positive organisational culture.
- Actively participate in UHHSOS planning and evaluation practices.
- Provide outreach service from Singleton and Upper Hunter hubs on rostered basis.
- Support roster at domestic violence accommodation run by UHHSOS
Selection Criteria
Essential
- Tertiary qualifications in a relevant discipline or demonstrated equivalent competencies gained through a minimum of three years relevant experience
- Demonstrated experience in working with people from culturally and linguistically diverse backgrounds.
- Knowledge and Understanding of the Child Protection System
- Excellent written and verbal communication skills, including the ability to advocate and resolve conflict
- Demonstrated skills and experience as a ‘best practice’ case manager. Including working in a strength-based, solutions focused, trauma-informed practice.
- High level of computer skills
- Practice respectful and ethical behaviour at all times
- Ability to work independently and as part of a team.
- The ability to critically reflect on professional practice and attitudes.
- NSW Working with Children’s Check.
- A current Driver’s License
- A current First Aid Certificate or a willingness to obtain one.
