Speak Up Google Form Instructions

It is super simple to create your own Speak Up customized log-in form!

Just log in to google forms (you will need a gmail account) at https://docs.google.com/forms

1.  At the top right click to create a blank form

2.  Add in your question fields depending on what information you would like to collect

a.  We suggest adding name as the first question – it will automatically change to short answer text as the question type

3.  Next format the template

a.  Add your title

i.  Example: [District or school name] Speak Up!

b.  Add a description if you would like

i.  Example: We encourage all teachers to Speak Up about digital learning this year! Join the conversation about the use of technology for learning through Speak Up, a national research project. Not only will your views be taken into consideration by our school, they will also be included in the national and state discussions on digital learning policies, programs, and funding. Fill out the form below to begin the survey.

c.  Click on the color palette to select the color of your form

d.  Add school or district logo

4.  Click on settings button

a.  Under General: Unclick the checkbox for Response receipts

b.  Under presentation

i.  Unclick “show link to submit another response”

ii. For confirmation message enter: Please click here to take the survey: [Enter in the direct start link for the specific school and audience e.g. teachers.]

c.  Click on the send button, click on the link button to get your link! Option to also send emails or embed code as well for your website

5.  I would do a quick test to make sure your form is doing what you want you can click on the preview button and it will show your form, fill it out. Make sure the correct link is on your confirmation page and check that your response was recorded.

Settings Options:

General tab:

Unclick response receipts, unneccary for this type of form.

Presentation tab:

Enter in your confirmation message here. You can either link the participant to their appropriate school and survey direct link or use the general link https://speakup.tomorrow.org/ChooseSurvey.aspx.

Send Form button:

Click on the icon in the “send via” row to either send by email, copy a URL link or get the embed code to place on our website.
