Transport Outreach Program Contract Amendment – January 2015

desires to participate in the Vanderburgh Humane Society (VHS) Spay & Neuter Clinic Transport Outreach Program. The following information outlines the terms of this agreement and the responsibilities of the party mentioned above (referred to as the transport group) and the VHS Spay & Neuter Clinic.

Transport Scheduling

·  The transport group agrees to assign a Transport Coordinator to schedule transports with the VHS Clinic’s Manager or Receptionist.

·  Transports can be scheduled in the following two ways:

o  Regularly-scheduled transport

§  Can be scheduled monthly, bi-monthly, weekly, etc.

o  Specifically-requested transport

§  Need to be scheduled at least 1 month in advance

§  Regularly scheduled transports will take priority over these

·  Transport groups are responsible for all marketing, advertising, and appointment scheduling.

·  The VHS Spay & Neuter Clinic requires a minimum and maximum number of animals for transports. Transports are free of charge when the minimum number of animals and points are met. The VHS Spay & Neuter Clinic’s schedule requirements are listed below. Failure to meet the minimum number of animals may result in the cancellation of the transport or additional transport fees. Repeated inability to meet transport requirements may result in the termination of the transport agreement and contract.

o  25 miles or less (one way)

§  15 animals is minimum.

§  Maximum numbers are at the VHS Clinic Manager’s discretion, but no more than 35 animals will be scheduled unless special arrangements are made.

§  If the minimum number is not met, a $25.00 transport fee will apply.

o  26-90 miles (one way)

§  25 animals is minimum.

§  Maximum numbers are at the VHS Clinic Manager’s discretion, but no more than 35 animals will be scheduled unless special arrangements are made.

§  If the minimum number is not met, a $50.00 transport fee will apply.

o  Over 90 miles (one way)

§  Special circumstances apply; arrangements must be individually made with the VHS Clinic Manager.

·  The transport group must notify the VHS Spay & Neuter Clinic of the total number of animals scheduled three (3) business days prior to the scheduled transport.

o  The transport group will either fax or email a copy of the transport sheet to the VHS Clinic Manager at (812) 426-2563 or .

o  If the minimum number of animals has not been met, the VHS Clinic Manager will determine at this time if the transport will be cancelled or if additional fees will be applied.

Schedule Requirements

·  The ratio of males to females shall be 1:1. In other words, there shall be equal numbers of males versus females.

o  Extra male cats are allowed. The total number of extra male cats is at the VHS Clinic Manager’s discretion.

o  If the transport consists of feral cats where the cats cannot be handled to determine the sex, this rule is exempt. All efforts should be made to determine a sex, unless doing so may cause harm to the caretaker and/or cat, or may result in losing the cat out of the trap if the cat is truly feral.

·  If the transport consists of both dogs and cats, no more than half of the total animals shall be dogs.

·  If the transport consists of dogs, no more than 4 of those dogs shall be 50 or more pounds in weight.

·  No more than 50% of a transport can consist of trapped cats.

Forms and Supplies Needed

·  The VHS Spay & Neuter Clinic will provide transport groups with a template of a current price sheet/fee schedule and a template of a flier for display.

o  Transport groups may copy and use these for distribution.

o  NOTE: The VHS reserves the right to approve all marketing materials advertising the VHS.

·  The VHS Spay & Neuter Clinic will provide templates of the dog and cat transport sheets at no cost to the transport group to keep track of scheduled appointments.

·  Admission forms will need to be obtained from the VHS Spay & Neuter Clinic.

o  The first 100 forms are free, but additional forms will need to be purchased at a cost of $40.00 per 100.

§  All admission forms must be completed. Without an owner’s signature, the VHS Spay & Neuter Clinic cannot legally do the surgery.

§  Every animal must have an admission form. If the owner has two or more animals, an individual admission form is required for each animal.

§  Admission forms must have owner signatures since this is the owner’s permission to complete the requested services.

·  Failure to collect a signature may result in that animal not receiving surgery and services.

·  The VHS Spay & Neuter Clinic requires waivers on all patients who will be receiving heartworm testing and feline heartworm, leukemia, and feline immunodeficiency virus testing.

o  The VHS Spay & Neuter Clinic will provide templates of the waiver forms at no cost to the transport group.

·  The VHS Spay & Neuter Clinic requires all animals scheduled for surgery wear a disposable name collar except for cats in traps.

o  The transport group is responsible for purchasing these, filling them out, and placing them on each animal.

§  There are available for purchase through the VHS Spay & Neuter Clinic for $40.00 per box.

o  Each collar should have the following written on it: the animal’s name, the owner’s last name, and the shelter/transport group’s name. If the animal doesn’t have an owner, a shelter tracking number is required.

·  It is the transport group’s responsibility to have enough carriers for the transport.

o  All carriers should be lined with newspaper and be marked with the following information: the animal’s name, the owner’s last name, and the shelter/transport group’s name. If the animal doesn’t have an owner, a shelter tracking number is required.

·  The VHS Spay & Neuter Clinic will provide post-operative instructions for each animal at the time of transport return.

Eligibility Requirements for Surgery

·  Clients should be asked if their animal has any prior medical concerns.

o  Prior medical concerns should either be declined or discussed with the VHS Clinic Manager.

·  Surgery can be performed on pregnant or in heat females, but owners should be informed of a slightly higher risk during surgery.

·  Advanced age is also a concern; surgery will not be performed on animals over 8 years of age, and animals 6-8 years of age must have a copy of a pre-anesthetic blood panel completed by a licensed veterinarian.

o  This must be completed within 2 weeks of the surgery date.

o  VHS offers this service for a minimal fee.

o  This rule is exempt for feral cats where the cats cannot be handled.

·  The VHS Spay & Neuter Clinic asks to be notified if female animals were strays because this can mean that spays could possibly be verified without surgery.

·  Animals must be of a healthy weight and not significantly over or under weight.

·  All animals must be healthy and free of signs of illness.

·  If any member of a littermate or kennel/cage mate is showing signs of illness, none of these animals will have surgery.

·  Cats

o  Cats/kittens must be at least 8 weeks of age and 2 pounds.

o  Nursing females must wait 2 weeks after kittens are weaned.

§  Females that are engorged with milk may need to have surgery postponed if the midline of the abdomen cannot be visualized.

o  Feral cats must be in a live/humane trap.

·  Dogs

o  Standard size dogs/puppies should be at least 8 weeks of age.

o  Toy breed dogs/puppies should be at least 12 weeks of age.

o  Nursing females must wait 1 month after puppies are weaned.

·  Animals 4 months of age and over must not have any food after 9:00pm the night before surgery but may still have water.

·  Animals younger than 4 months must have food and water.

·  If there is any question that the animal may have had food, owners will be asked to reschedule the surgery.

·  The VHS Spay & Neuter Clinic Veterinarian will make the final determination if an animal has met all of the eligibility requirements for surgery.

·  Failure to abide by all requirements, in additional to other issues noted at the time of a physical examination may prevent the animal from having surgery.

VHS Spay & Neuter Clinic Service Requirements

·  The VHS Spay & Neuter Clinic requires all patients 12 weeks of age and over to receive a rabies vaccination if a current rabies vaccination cannot be verified.

o  It is the transport group’s responsibility to require proof of a current rabies vaccination. Rather than through a tag, proof must be verified through a rabies certificate that verifies that that animal was the one vaccinated and the date the vaccine was given was within the past year.

§  The transport group shall initial the admission form when proof has been provided. A copy of the rabies certificate must be attached to the admission form.

o  If proof cannot be provided, a rabies vaccination will be given, and the owner will be charged for this service at the time of service.

·  The VHS Spay & Neuter Clinic requires all patients receive a green tattoo on the animal’s underbelly near the incision site to identify the animal has been spayed/neutered.

·  The VHS Spay & Neuter Clinic requires all feral cats to receive ear-tipping to identify from a distance that the animal has been spayed/neutered.

Services/Fees and Invoicing

·  The transport group is responsible for determining the fee and payment structure for their community.

·  The VHS Spay & Neuter Clinic’s transport pricing structure is as follows:

o  All prices are subject to change. The VHS Spay & Neuter Clinic agrees to give transport groups a 30 day notice prior to price changes.

·  when the veterinarian has determined euthanasia to be the most humane option for a severely injured or ill trap cat and an owner has signed a waver giving permission for euthanasia.
Anesthesia without surgery
(Induction) / $25.00 / This fee applies when animals are anesthetized and do not receive surgery due to unforeseen health concerns, or sterilization of patients can be verified without surgery through scars, tattoos, etc.
Pre-operative blood work / $30.00 / Includes physical exam. This is required to be completed before the scheduled surgery date for animals 6-8 years of age.
Microchip Implantation / $5.00 / This fee is due to the cost of biohazard disposal, extra staff time to implant and extra paperwork.

·  The transport group is responsible for accepting payments from owners for all fees incurred. Handling money and having change available are the total responsibility of the transport group. It is advisable to have all services prepaid with cash or money order to avoid nonpayment of services.
