SNAP-Ed Partnership Agreement

Between:Solid Ground’s Cooking Mattersand name of Partner (Party B)

Purpose:

The purpose of this partnership agreement is to clearly identify the roles and responsibilities of each party related to providing Supplemental Nutrition Assistance Program Education (SNAP-Ed) to*eligible persons in Washington state. (*Eligibility is determined based upon currently receiving or able to receive SNAP benefits in Washington or data which substantiates eligibility of a population to be served.)The Supplemental Nutrition Assistance Program Education (SNAP-Ed) program is intended to provide nutrition education to low-income individuals.

To support this goal:Solid Ground’s Cooking Mattersand (Party B)will collaborate to assure that eligible persons have access to available nutrition education interventions.

  • Both parties will assure that nutrition and physical activity messages are consistent with the 2010 US Dietary Guidelines for Americans and MyPlate.
  • Both parties will strongly encourage and support the involvement of nutrition and physical activity education by other organizations in their communities.

This local Partnership Agreement may include, but is not limited to the following:

Solid Ground’s Cooking Matters Agrees to:

  • Provide and deliver the SNAP-Ed intervention, i.e, the approved Cooking Matters curriculum, as well as materials and supplies.
  • Record the necessary data, evaluate and maintain records.
  • Provide taste-testing samples for participants of the SNAP-Ed intervention/Cooking Matters curriculum.

(Party B)Agrees to:

  • Promote the SNAP-Ed intervention/Cooking Matters curriculum to new eligible participants.
  • Support the SNAP-Ed staff delivering the curriculum by allowing staff to engage in the SNAP-Ed intervention/Cooking Matters curriculum.
  • Inform SNAP-Ed/Solid Ground Cooking Matters staff of any policy or rule changes that may related to nutrition education for your site.
  • Provide an appropriate space and learning environment for use during the SNAP-Ed intervention/Cooking Matters curriculum.

Funding:

Federal USDA SNAP-Ed funds are not to be used to supplant existing nutrition education funds or activities.Funding for each fiscal year is subject to approval by USDA.If full funding does not become available, this agreement may be amended, reduced or canceled anytime.

Effective Date Signatures:

This agreement is effective on the date signed by all parties and valid from October 1, 2015 to September 30, 2016.
