This agreement constitutes permission to provide day care and boarding services for said animal.
Any changes to this agreement must be done so in writing or they will be null and void. Small’s K-9 School & Rescue has the right to make any changes to this agreement at will and 24 hour notice to the client. With any changes, a new agreement will be presented before any new services are rendered.
I, Luiza Small ask that you bring toys, blankets, bed and crate (if used-I, Luiza Small do prefer you have a crate to bring). However I am (Luiza Small) will not be responsible for these items. Please bring food with instructions and mark food with your dog’s name and feeding instructions. Do the same with any medications, and daily routine (I, Luiza Small will try to follow is as close as possible).
Because of the type of boarding I, Luiza Small do it’s essential that you drop off and pick up on time. I,Luiza Small also require you to call 30 min prior to drop off and pick. This is my home and although I, Luiza Small am set up for dogs it is more like your dog is coming to a private home. I, Luiza Small want to make sure it’s a positive and fun experience for them. I, Luiza Small also ask that if you’re going to be running late that you call me and let me know!!!! Thank you for understanding.

Veterinarian Release Form
I, ______, being the owner of ______(dog’s name), hereby give any and all authorization to Small’s K-9 School & Rescue/ Luiza Small to make needed health decision regarding my dog’s well being.
By executing this document I acknowledge thatSmall’s K-9 School & Rescue/Luiza Small will be able to make any necessary decision regarding my pet’s medical needs in my absence. I also give Small’s K-9 School & Rescue/Luiza Small my authorization to get any, and all available medical records from______veterinary clinic.

I, Luiza Small must have a credit card on file in case of a Veterinarian emergency or you need to make other arrangements with your vet clinic.

Because I, Luiza Small am a small facility I (Luiza Small) am often booked in advance, fees are nonrefundable. You can give me a check, however by filling this out and signing this contract you agree to pay the boarding fees if you cancel. If you need to make changes please do so at least 48 hours in advance. Fees are paid when you drop your dog off so if you are paying by check please bring a check made out to Luiza Small Re: Small’s K-9 School & Rescue.
WE must arrange your pick up time in advance or you will be charged an extra stay.
Policies and Procedures
Of Small’s K-9 School & Rescue Boarding Services
Please read carefully and initial next to every point in the space provided.
The “Client” or “I” agrees to the following:
I______understands that my dog must have been seen by a veterinarian and must be current with all vaccinations. Vaccinations must be given far enough in advance to be effective. I understand thatLuiza Small stronglyrecommends vaccination against kennel cough, which is very common among dogs and can be contracted anywhere dogs meet. Please bring vaccination record with you before or during drop off or pet will not be admitted.______
I understand that my pet must be treated with a flea, tick and heartworm preventative. If fleas are found on my pet, a flea bath will be given at my expense to my dog and all animals in the residence of Luiza Small at the time.______
I understand that Small’s K-9 School & Rescue does not offer veterinary services, any veterinary or medical concerns will be referred to a veterinarian at the owner’s expense.______
I understand that my dog will be interacting with other animals. My dog hasNO known aggression problems and is free of any transmissible diseases.______
I amsolely responsible for any harm caused by my pet to anyone, other pets, owners, or property.______
I understand that if my pet is found to be aggressive or displays behavioral problems it will no longer be allowed to come and stay at Luiza Small’s residence and must be transferred to another kennel at my own expense.I understand that Small’s K-9 School & Rescue/Luiza Small will not refund moneys for unused days due to a serious behaviour or aggression problems. ______
I understandSmall’s K-9 School & Rescue/Luiza Small does not accept aggressive animals since all dogs will be interacted with other animals, people and children living in that residence. If my dog attacks another or bites anyoneI will be responsible for any and all damages (If you’re unsure how your dog will be with other animals please let me, Luiza Smallknow and we’ll discuss it. This is very important and can be quite costly in the event of an attack).______
Small’s K-9 School & Rescuewill not walk unruly or untrained dogs or dogs that choke themselves on their leash. All dogs must be accompanied by a workingleashor they will not be admitted. Extra training can be done upon request for extra charge (needs to be requested).______
I understand that Small’s K-9 School & Rescuereserves the right to take my dog to the vet if necessary. I,the client will beresponsible for all charges.______
Small’s K-9 School & Rescuereserves the right to deny service or terminate service because of safety concerns, or inappropriate or uncomfortable situations.
All paymentsare expected before services are rendered. In the event of additional unforeseen costs (such as vet fees), payment is expected within 5 days of the completion of services or a late charge of $50 will be applied.
Please know that if the dog is not picked up on the third business day after date of pick up stated above, the dog will be considered abandoned and can be re-homed. By executing this document I,______acknowledge that I will not be able to reclaim this animal once it is in the possession of Small’s K-9 School & Rescue unless I advise Luiza Small of late pick up in writing.
By signing below I fully understand and agree to the contents of thisagreement.

This agreement is in effect for all services rendered by Small’s K-9 School & Rescue or any other party that is associated with Small’s K-9 School & Rescue for Dogs for 12 months from date of agreement.
Release and waiver of Liability and assumption of risk
I,______understand that I am taking certain risk of damage or injury to myself, my guest(s), my property, my dog and to others. I realize that when dogs are around people and other dogs, some accident or injury may result, and that people, dogs and property might be injured or damaged.
In exchange for the opportunity to use Small’s K-9 School & Rescue and its facility, I release and discharge Small’s K-9 School & Rescue/Luiza Small and anyone associated with this business from any and all liability, claims, demands, causes of action, loss damage or injury to person or property, including death and serious injury which may be caused by myself, my guest(s) or my dogs(s) while they are on the premises of Luiza Small residence or attending and event sponsored by Small’s K-9 School & Rescue.
I hereby confirm that I am eighteen (18) years of age or older and have legal capacity to enter into a binding contract under the laws of Province of Ontario. I intend for this release to be binding upon my heirs, beneficiaries, next of kin, personal representation or executor, and assignees.
I have read, understand, and agree to abide and be bound by the terms and conditions of Small’s K-9 School & Rescue agreement and contract. I understand that Small’s K-9 School & Rescue reserves the right to alter or amend the rules at any time with 24 hours prior notice.
