Health Care Proxy
The Narris Law Office will provide you with a Health Care Proxy. Your health care proxy (or agent) is someone you trust and name to make medical decisions for you in the event that you are unable to communicate for any reason, at any time. You can name your spouse. You can also name an alternate, in the event that your spouse is unable.
The person you name must be 18 years old.
Keep the original of your health care proxy, but in a place that your loved ones will have easy access. (Not in a safe deposit box, where it is advisable you keep your Will).
Make copies of your health care proxy and provide one to your doctor, family or anyone else that would be involved in making decisions for you.

Acknowledgment
This intake form is for the drafting of a “simple will.” This form contemplates that your entire estate will go to your spouse or to your children in equal shares, if your spouse predeceases you. If this is not the arrangement that you wish, please tell the attorneys at the Narris Law Office.
If you are drafting this will with your spouse and are represented jointly by the Narris Law Office, you may disclose otherwise confidential information. This could create a conflict of interest for the attorney. If you would prefer to have another lawyer draft your Will separate from your spouse, please make sure to inform your attorney. GENERAL INFORMATION
Purpose of a Will
By drafting a Will, you can provide your family with specific instructions about your wishes. This can abate any contentiousness for your family on how to handle your estate and care of your children. A Will can provide you with tremendous peace of mind that your loved ones will be cared for.
The other reason that people draft a Will is to anticipate estate tax implications. If your estate includes property that evokes the Federal or State estate taxes, you may wish to seek the help of a financial estate planner to decide whether you should also create various trusts in consideration of possible taxes.
Signing of the Will
In order to sign the Will, you will need to have two witnesses present. It is advisable that these people should not be “interested parties,” or people that stand to inherit from your Will.
Each person signing the Will and also witnessing the Will must be present at the same time. Each person should bring a valid form of ID so that their signatures can be notarized.
The Attorneys at the Narris Law Office will schedule the signing at a time and place that is convenient for you and the witnesses.
Storage of your Will
After your Will has been drafted and executed, it is advisable that you keep the original in a safe place. Provide information to you family about where they can find your Will and access important information. The Narris Law Office will retain a copy of your Will.
People oftentimes keep their Will and documents of non-probate assets (life insurance policies, trusts, etc.) together. We advise that clients keep these documents in a safe deposit box. Keep in mind that it can be time consuming to access a safe-deposit box if the person is not also named on the box. It is also advisable to keep a description andlocation of your various properties and important paperwork, and to make sure your personal representative knows where to find this information.
Basic Planning
You can also write informal letters of instruction about your funeral arrangements or desires for your children’s future and give those letters to your family members and/or keep a copy with your estate planning paperwork. Be sure to tell your family where you keep these important papers so they are easily accessible. You also are able to fill out an organ donor form to reflect your wishes. These forms are available online or can be provided to you by the Narris Law Office.
