Dear Student,

Park Nicollet requires all students report their flu immunization status.This means you must report the date you were vaccinated or reason you are declining the flu immunization. Reporting has been optional in the past, but is now mandatory.

No computeraccess?To verbally report you decision, you can stop by EOHS in Meadowbrook W515 or call 952-993-5080, option 2. Give your name, date of birth, date and place you were vaccinated or reason you wish to decline. Declination reasons include:

  1. I am severely allergic to eggs.NOTE: EOHS can now order a dose of flu vaccine that is egg free. Email or call 952-993-5080 to request
  2. I have I am allergic to a component of the influenza vaccine.
  3. I have another medical reason for not receiving the influenza vaccine. ______

EOHS is offering FREE flu immunizations to students. Stop by EOHS located in Meadowbrook W515 anytime Monday - Friday between 7am-4:30pm to receiveyour flu immunization. No appointment is needed!

Why should students get a flu immunization?

The Advisory Committee on Immunization Practices recommends that all persons six months of age and older should be vaccinated annually against influenza. Persons who are infected with the influenza virus, including those who do not yet have symptoms, can transmit the virus to coworkers and patients, including those at higher risk for complications from influenza. Vaccination of adults has been associated with reduced risk of influenza illness, and reduced antibiotic use and medical visits. In addition, vaccination has been found to reduce deaths among elderly hospitalized patients.
