SERVICE PROVIDERS – My Service Logs and Student Service Logs

People who are assigned as Service Providers are able to create draft information to be included in the IEP/IIIP/IFSP for that student. The Case Manager can merge draft information into documents from all contributors, allowing them to work collaboratively.

Although Service Providers cannot edit other information for the student unless they have been given those permissions specifically, they are able to access other documents that have been created for the student.

My Service Logs

People who have been assigned the role of Service Provider in the Student Plans system may have a My Service Logs available to them on the left Menu Bar.

On the Menu Bar, click My Service Logs. / The My Service Logs page is displayed.
Click the links at the top of the table to change the view. / This page defaults to showing Service Logs for the Past Week. You may also select Past Month, Past Year, or All.
Click the link in the Status column to select the student whose documents you wish to work on. / The Status column indicates the status of the IEP/IIIP.

Student Service Logs

People who have been assigned the role of Service Provider in the Student Plans system are able to create Service Logs for students whose plans have been Finalized in the Student Plans system. Service logs are only visible to the Service Provider and the Billing Manager.

On the Menu Bar, click My Services. / My Services page is displayed.
Next to student name, click link to select a service for a student. / The Service Logs page is displayed.
Note that Jordan does not have any service logs. This page defaults to showing Service Logs for the Past Week. You may also select Past Month, Past Year, or All.

Click the Add New Service Log link. / The Service Log page is displayed. Click the links at the top of the table to change the view.
Click in the radio buttons and text boxes and fill in all of the required information. Keep in mind that the log date must be between the service start and end date. / The items labeled with red text and an asterisk (*) are required information that must be filled in if they’re not already.
Click the Submit button. / The Service log is redisplayed with a success message.
Click the Cancel button. / The Service Logs page is displayed with the item you just created added to the table.
The service log can now be edited or deleted.
