Sequence for Search Reports

Submit Search Forms to theOffice of Human Resources

Form 1 - Screening Committee Roster- submit when the committee has been formed and a chairperson has been named for the search.

Form 2 - All Applicants–this is done automatically by the Online Application System. The Equal Opportunity Office has access to the All Applicant’s List with the Confidential Data submitted by the applicants.

Form 3 - Unqualified Applicants– List the applicants on this form who do not meet the required qualifications, submit with Form 4.

Form 4 – Qualified Applicants. List all of the applicants whomeetthe required qualifications as listed in the position announcement. ** Form 3 and Form 4 should be submitted together. Now STOP! IMPORTANT- POOL APPROVAL IS NOT REQUIRED FOR LOCAL/REGIONAL SEARCHES BUT THE EO OFFICE REVIEWS FORMS 3 – 6A PRIOR TO SETTING UP INTERVIEWS. POOL APPROVAL IS STILL REQUIRED FOR NATIONAL SEARCHES; TENURE TRACK FACULTY AND HIGH LEVEL ADMINISTRATIVE POSITIONS.

National Searches – After the submittal of Form 3 and Form 4, the Office of Human Resources will submit a report to the appropriate Hiring Authority and requestan approval of the pool. When the approval has been received, the committee chair will be notified by the HR Office that the committee can proceed with the qualitative screening process and submittal of: Form 5 – Qualified but Not Interviewed and Form 6 – Interviewee’s Report (Telephone/On-Campus).

Form 5 - Qualified-Not Interviewed- submit Form 5 AND either Form 6A – Phone Interviews or 6B – On Campus Interviews for review by the HR Office PRIOR to the start of interviews. This is the HR Office’s opportunity to review the pool of candidates to see if there are any qualified women, ethnic minorities, veterans or individuals with disabilities on Form 5 and determine if a review of these candidates is necessary. Form 6 – Interviewees Report – This is a list of applicants the committee would like to interview with an explanation of why they are the best qualified candidates for the position; factual statements about skills, education, and experience as related to the posted position announcement using the required and preferred qualifications. Phone Interviews (Form 6A) - use this form if you would like to qualify applicants first by phone, be sure to include individual “summary statements” on the second page of this form. On Campus Interviews (Form 6B) - use this form to clarify who the committee would like to bring forward for on-campus interviews, fill in the second page of the form with individual summary statements.

Form 5 and Form 6A and /or 6B should be submitted simultaneously for review. Do not forget individualized summary statements for each candidate on Form 6 (A and B) Interviewee’s Report (second page). The Office of Human Resources will notify the committee chair when they can proceed with the interviews when these two reports have been submitted and reviewed.

Form 7 Recommendation/Selection Report with Summary Statements- The HR Officer will review Form 7 Selection Report and the search file. A Certification of Search is sent to the Search Chair and Hiring Authority that states they have followed the MSU Denver Search and Screening procedures and may move forward with the salary assessment, background check authorization, and offer of the position.
