The questionnaire is a self-evaluation tool designed to identify strengths and weaknesses of participants who attend the storyteller training. In addition, the questionnaire is a needs assessment that allows for participants to specifically request future training.

Please rate your ability to complete a task by circling the number that best describes your ability.

1 = Yes, I can complete the task with confidence and without assistance.

2 = I can complete the task, but might need assistance or review of training content.

3 = No, I cannot complete the task at this time and would need further training.

There are four parts in the needs assessment. After completing each part, take a moment to reflect on what you have learned and on what you identify as needs for future training.
