Witnesses and parties must call the toll free number at 1-855-750-1027 and enter thePIN Number: 091262 to participate in the scheduled public input hearing on July 10, 2013 at 1:00 p.m. Beginning at 1:00 p.m. on July 10, 2013, witnesses will testify in the order they signed up. Any persons that have not contacted the Office of Consumer Advocate by July2, 2013, will not be able to participate in the telephonic public input hearing.

If a witness has any hearing exhibits to which he/she will refer during the public input hearing, (3) three copies must be sent to the Administrative Law Judge and 1 copy each must be sent to every other party on the service list attached to this notice.
