  • A clear description of the nomination, in no more than 400 words, providing a real sense of the objectives, the performance delivered, the individual or people involved, and the successful achievements or positive impact.
  • An overview of the activity involved
  • Evidence of the behaviours and ways of working which contributed to the success
  • What makes this nomination special and deserving of an award
  • It is essential that you provide information about the activity and impact

Category 1. Promoting choice, control and person centred approaches for people with a sensory loss
  • Creative ways of planning and implementing services alongside people who have a sensory loss and also involving their families and friends
  • Solutions focused approaches
  • Individualised services demonstrating a move towards services built and delivered around individuals

Category 2. Personal and workforce development
  • Creative ways of increasing individual and workforce capacity and capability
  • Examples of empowering and facilitating people with a sensory loss to achieve their potential
  • Ensuring that services are operating within a framework of continuous improvement

Category 3. Promoting inclusion and engaging people with a sensory loss across all our communities and services
  • Involvement of people with a sensory loss in the design, development and delivery of services, events or projects
  • Use and promotion of inclusive communication
  • Championing and promoting access for people with a sensory loss to mainstream services and facilities
