SCL Incident Reporting Process

Incident Report

Notify guardian and record time (within 24 hours)

Notify Case Manager and record time (within 24 hours)

Have employee involved fill out page 1 and 2 of the incident report form immediately

Within 10 days fax completed report with follow-up to Case Manager and attach fax cover sheet and fax report. Be sure to include in the follow-up how the case manager was involved.

If a medication error include in follow-up name of medication, number of times per day, and dosage

File report in both the IR Central File AND the Individual’s Binder

Critical Incident Report

Notify guardian and record time (within 8 hours)

Notify Case Manager and record time (within 8 hours)

Have employee involved fill out page 1 and 2 of the incident report form immediately

Notify DDID by faxing report to 502-564-2284 (within 8 business hours)

Within 7 days fax completed report with follow-up to Case Manager and attach fax cover sheet and fax report. Be sure to include in the follow-up how the case manager was involved.

File report in both the IR Central File AND the Individual’s Binder

Critical Incident Report (abuse, neglect, exploitation)

Notify guardian and record time (immediately)

Notify Case Manager and record time (immediately)

Have employee involved fill out page 1 and 2 of the incident report form immediately

If the individual is in immediate danger call DCBS (Department for Community Based Services) at 859-245-5258 or their afterhours line at 1-877-597-2331

If they are not in immediate danger fill out the online form immediately and submit at https://prd.chfs.ky.gov/ReportAbuse/home.aspxEmail only if it is between the hours of 8AM-4:30PM Mon-Fri and it is not an emergency.

Notify DDID by faxing report to 502-564-2284 (within 8 hours). Include APS email confirmation if submitted through website.

Within 7 days fax completed report with follow-up to Case Manager and attach fax cover sheet and fax report. Be sure to include in the follow-up how the case manager was involved.

File report in both the IR Central File AND the Individual’s Binder

Indicate how CM was involved in the follow-up of the incident

Remember to include relevant information about what happened before the incident and what happened as a result of the incident

Write in complete sentences

Include full names of staff involved
