We hereby apply for the students listed on the reverse side to participate in the “Tour of Opportunities” facilitated by Reconciliation South Australia and partnering organisations:

We understand that the Tour of Opportunities will cater to groupsof students (ranging in optimum size of 5-10) supervised by a teacher. Schools can send more than one group according to their preferences(please fill out an application form for each group).

We confirm that the required and appropriate processes used for excursions have been completed by our school and all the necessary consent forms from parents -including permission to take and use photos of their children for promotional purposes - and relevant insurance documents have been obtained by our staff.

In the table below, please choose the industry types (listed to the right) according to each group’s preferences(1 to 5: 1=top preference; 2=second etc.).

Every effort will be made to match student groups to their preferred cluster choices, subject to availability.

The following table summarises all the relevant information about our students and we acknowledge that the role of Reconciliation South Australia is only the facilitator of the program and bears no duty of care responsibility for the students. Our school will provide a duty of care teacher to accompany and supervise the students (1 to 10 ratio) listed in the table below. In the instance that a parent / caregiver is to accompany a student we will advise the organisers accordingly.
