Explanation of Chiropractic Medicare Benefits
Medicare does cover chiropractic care but with the following limitations:
Medicare prefers you have current x-rays of your spine and the x-rays must show a subluxation. Medicare does NOT cover the cost of the x-rays, although Medicare requires them. Other non-covered services include but are not limited to: examinations, therapies, supports, supplements, adjustments on extremities of the body (ex: arms, legs, etc.), or other services offered in this office.
After the yearly deductible has been met, the only services covered by Medicare are the manual manipulations of the spine. Under some circumstances and with some Medicare carriers, these manipulations are limited.
Medicare will only pay 80% of the manual manipulation after the yearly deductible has been met. The 20% coinsurance, x-rays, examination, therapies, supplements, supports, adjustments on extremities of the body (ex. arms, legs, etc.), and any other visit over your limitation within the year will be your financial responsibility.
If you have a secondary insurance, they may pay for the deductible and the 20% co-insurance of the manual manipulation. However, they will NOT cover ANY services denied by Medicare, such as therapies, x-rays, examinations, supplements, supports, or any other services offered in this office.
The below signature acknowledges that I have read the above statement and understand the policy and financial responsibility. I also authorize direct assignment of payment for all professional services to be paid by my insurance to Schmidt Chiropractic Center located at 320 Hill St, PO Box 215, NYA, MN 55368, and realize that any balance after my insurance will be promptly paid.
Also, I further agree to allow a release of any and all medical records to my health insurance, if requested, in order to insure prompt payment on the medical claim.
