All SRV STUDENTS must complete this form to attend the BALL.

______(Name of SRVHS student) has my permission to attend the Senior Ball at AT&T Park, on the evening of May 12, 2012. I understand that my son/daughter will be responsible for his/her own transportation to and from AT&T Park in San Francisco, however luxury buses are available and seats can be purchased with Ball tickets (information on info sheet). The dance begins at 7:30 p.m. and ends at 11:30 pm.

Behavior at school dances will be closely monitored. Possession of, or being under the influence of any controlled substance is prohibited. Guests in violation will be asked to leave the dance and the police will be contacted. A Breathalyzer may be used at the discretion of the administration.

Doors will be closed at 9:00 P.M. (Students may be admitted after 9:00 P.M. with prior permission of the administrator in charge).

Guests must be accompanied by a SRVHS student and must be a current high school student to attend the SRV Prom. SRVHS students and guests must show a current school I.D. Any exceptions to these rules must be approved by the SRV administration before ticket purchase.

There are no “in and out” privileges at dances. Once a student exits the dance he/she will not be readmitted.

Students or guests will not be admitted if their attire is too revealing or inappropriate for a high school function. The administrator in charge will determine what is appropriate. Attire that is low cut or exposes the midriff is not appropriate. Dresses can be shown to administrators ahead of time if there are concerns.

Students and guests must dance in a safe and appropriate manner. Demeaning or sexually explicit dancing, groping, fondling, bending over, lap dancing, floor break dancing, and moshing are not acceptable.
