Sample Pick Up and Transport Protocol

  1. Sample Collections
  2. Clinicians and facility will package their samples, using the proper packaging protocol reference in S-OIV Testing Protocol Element 2 also visit .
  3. Clinicians and facilities will log in the number of samples and patient counts on sending sample manifest 1. See attachment (A)
  4. Sending sample manifest 1 accompanies the samples to the designated participating hospital lab(s).
  5. Initial Sample Delivery
  6. Clinicians and clinics will deliver the sample to the designated receiving hospital lab section. See Attachment (B) for list of designated receiving hospitals.
  7. Hospital labswill receive and refrigerate the properly packaged samples from the Clinicians and facilities for storage purposes only. Note: There is no chain of custody required to receive the samples.
  8. Sentinel physicians are to call 1-877-611-6883 to arrange pick up of their samples.
  9. Sample Pick Up
  10. DHH Staff picks up samples from designated receiving hospital’s lab and sentinel physician. See Attachment (C)
  11. Regional pick up time will be scheduled to coincide with DHH Relay Destinations departures
  12. DHH Relay Destination A
  13. Sample from Region 1,2,3,4,5 and 9 will be brought to OPH Regional Offices. See Attachment (D)
  14. DHH Relay Destination B
  15. In Regions 2, 4 and 5 one DHH employee, from each region, will merge the samples into one shipment and transport samples to the DHH-EOC relay point.
  16. Samples will be transferred to DHH Transport Pool personnel at the DHH-EOC for transport to the DHH State Lab.
  17. The number of samples received and the number of patients this represents will be noted at this time.
  18. DHH Sample Transportation Branch shall be notified by calling 1-877-611-6883 with the number of samples being transported to the lab, the number of patients this represents and the expected time of sample arrival the State Lab.
  19. State Police Relay Destination A
  20. Samples from Regions 6 will be brought to the State Police Troop Headquarters E.
  21. The employee delivering the samples to State Police will note the number of samples transferred to State Police and the number of patients this represents. This employee will notify the DHH Sample Transportation Branch by calling 1-877-611-6883 as to the number of samples received and the number of patients this represents.
  22. State Police Relay Destination B
  23. Samples from Regions 7 will be brought to the State Police Troop Headquarters G.
  24. The employee delivering the samples to State Police will note the number of samples transferred to State Police and the number of patients this represents. This employee will notify the DHH Sample Transportation Branch by calling 1-877-611-6883 as to the number of samples received and the number of patients this represents.
  25. State Police Relay Destination C
  26. Samples from Regions 8 will be brought to the State Police Troop Headquarters F.
  27. The employee delivering the samples to State Police will note the number of samples transferred to State Police and the number of patients this represents. This employee will notify the DHH Sample Transportation Branch by calling 1-877-611-6883 as to the number of samples received and the number of patients this represents.
  28. Samples Received by Lab (A)
  29. In Regions 1, 3 and 9 one DHH Transport Personnel per region will merge the samples into one shipment per region and transport samples directly to the DHH State Lab3101 West Esplanade BlvdMetairie, LA 70001
  30. The State Lab will contact the DHH Sample Transportation Branch by calling 1-877-611-6883 to verify receipt of the samples and the number of samples received, the number of patients this represents and from whom the samples were received.
  31. Samples Received by Lab (B)
  32. DHH Sample Transportation Branch shall be notified of the number of samples being transported to the lab, the number of patients this represents and the expected time of sample arrival the State Lab.
  33. The State Lab will contact the DHH Sample Transportation Branch to verify receipt of the samples and the number of samples received, the number of patients this represents and from whom the samples were received.
  34. Samples Received by Lab (C)
  35. State Police will take control of the samples collected in Region 6, 7 and 8 and will take responsibility for their delivery to the DHH State Lab. The employee delivering the samples to State Police will note the number of samples transferred to State Police and the number of patients this represents. This employee will notify the DHH Sample Transportation Branch by calling 1-877-611-6883 as to the number of samples received and the number of patients this represents.
  36. The State Lab will contact the DHH Sample Transportation Branch to verify receipt of the samples and the number of samples received, the number of patients this represents and from whom the samples were received.
