Program Description

The Substance Abuse and Mental Health Services Administration (SAMHSA), Center for Substance

Abuse Treatment (CSAT) awarded the Department of Social and Health Services (DSHS) - Division of Behavioral Health and Recovery (DBHR) the five year Screening, Brief Intervention, and Referral to Treatment (SBIRT) grant in September of 2011. Washington’s goals with this grant have been to disseminate, implement and sustain SBIRT services in the Primary Care settings throughout the State. The Washington State SBIRT-Primary Care Integration (WASBIRT-PCI)project is responsible for program implementation and monitoring; information dissemination; and creating a sustainable, reimbursable SBIRT program. SBIRT is an evidence-based public health practice based on universal alcohol and drug screening to increase awareness about an important preventable health issue - substance abuse.

Program Model

WASBIRT-PCIis currently offering SBIRT services to adults receiving primary care and emergent care in the following counties: King, Whitman, Cowlitz and Clallam County. The project integrates behavioral and primary health care and has worked to create a sustainable reimbursement system for SBIRT under Medicaid. In an effort to disseminate SBIRT broadly, WASBIRT-PCI funds clinic sites for 12-18 months. During this time we assist them with training, technical assistance, and creating a sustainable clinical practice. The intent of the project has been to disseminate SBIRT statewide by providing comprehensive training to grantee and non-grantee sites and develop protocols for SBIRT reimbursement to sustain the program statewide.

Special/Unique Features

Some unique features of WASBIRT-PCIinclude:

  • Mixture of grant funded clinics and hospitals that are urban and rural.
  • Funding an FQHC in King County who specializes in services for the Latino population.
  • Funding a clinic that is operated by Jamestown/S’Klallam Tribe, which serves over 300 tribal members.
  • Developed the Screening, Performance and Results (SPAR) electronic data collection tool for GPRA data collection, with cross-checks and validation standards that allows for timely, secure data transfer.
  • WASBIRT-PCI has facilitated partnerships between community emergency departments and primary care clinics to enhance the continuum of care for patients identified as having some level of substance use disorder.
