Nomination Form and Instructions

Southeast Alaska Independent Living’s (SAIL) Disability Rights Advocate of the Year Award recognizes the many individuals (both with and without disabilities), organizations and businesses whose work (paid and/or volunteer) enhance the dignity, quality of life and inclusion of people with disabilities and seniors throughout all of our Southeast Alaska communities. Recent honorees include: Cheryl Putnam (2012), Eaglecrest Ski Area (2014), Sara Boesser (2015), Allen Marine Tours (2015), Lynn Marvel (2016) and Tim Huber (of Haines, 2016).

Criteria for nomination selection include:

  • Candidate has demonstrated a commitment and resolve to furthering the causes of seniors and/or people experiencing disabilities;
  • Candidate has committed their work to advocating for individuals of all ages and abilities to live the lives they dream in the community and setting of their choice;
  • Candidate reflects SAIL’s core values of Empowerment, Accessibility, Integrity, Collaboration and Inclusion throughout the community;
  • Candidate’s work has represented and/or benefited diverse groups of seniors and people experiencing disabilities;
  • Candidate’s work can be paid (i.e. professional) or unpaid (i.e. volunteer);
  • Nominees may be seniors or individuals experiencing a disability, community members (both professional and volunteer), or a local business or organization. Anyone doing good work for seniors and people experiencing disabilities is a candidate!

Please use the form attached. Up to three letters of support may be attached (optional). We ask that each nomination be limited to two pages, citing specific examples of how your nominee meets the awards criteria.Please contact SAIL at 586-4920 if you need large print, alternative format or any other assistance/accommodation in filling out this form.

Honorees will be recognized at the annual SAIL Gala and Auction, Saturday, October 7th, at Centennial Hall.

Nominations are due Tuesday, September 12th at 5pm.
