Primary Objective:
The Community Nursing clinical staff is primarily responsible for assessment, planning, delivery and evaluation of safe and appropriate consumer centred nursing care in the community setting.
To actively participate in the activity required for all employees of CAH as set out in supportive policies, guidelines and procedures.

- Service Delivery

- Guidelines for the Provision of Hospital in the Home when assigned HITH clients.
- Works within the Framework for delivery of Care for Post Acute Care (PAC) Services when assigned PAC clients.
- Works within the BSW Framework for Community Palliative Care Services when assigned Palliative Care Clients.
- Demonstrates clinical expertise and problem solving ability, capable of working independently in the community setting.
- Demonstrates the ability to move between programs within Community Nursing as required.
- Demonstrates the ability to ensure client scheduling is managed effectively.
- Knowledge and understanding of the service coordination and service delivery guidelines: i.e. Home and Community care (HACC), Department of Veterans Affairs (DVA), and other funding agencies and the ability to transfer the theory into clinical practice.
- Establishes a collegial working relationship and work collaboratively, with multidisciplinary and interdisciplinary team members across the continuum of care.
- Liaise with relevant care providers; Community Nursing and Community Services workers and Acute and community services staff in order to build efficient and appropriate referral network and framework for consumers, carers and family members.
- Actively invite the involvement of the client as a partner in the multidisciplinary team, and ensure that outcomes from multidisciplinary care meetings are incorporated into the care plans of consumers.
- Acts a clinical resource within the team assists with the orientation of new staff members, and students as requested.
- Delivers prompt and courteous culturally appropriate service to the community that is consumer and carer centred care.
- Nursing documentation is completed in an accurate, legal and timely manner to reflect the Active Service Model client journey.
- Client contacts to be recorded electronically daily.
- Delivers service coordination, clinical assistance and support to consumers, carers and co-workers. Evidenced by consumer workload management, documentation, workload, appropriate referral and handover.
- Development and maintain effective communication and professional relationships with relevant internal and external stakeholders.
- Outcomes of care confirm effective quality nursing practice and demonstrate the application of an evidence based practice.
- Contributes to Community Nursing Programs/projects and reporting requirements through accurate and timely data collection and data entry.
- Adopts a partnership approach to organisational, individual and community resources including vehicle booking.
- Refers complex or unresolved clinical issues to Clinical Coordinator when rostered or if unavailable the Community Nursing Manager.
- Ensures set timelines and time frames are adhered to and any issues are reported back to Clinical Coordinator when rostered or in their absence the Community Nursing manager in a timely manner.
- Completes yearly mandatory Training and relevant competency packages within the required framework.
- Maintains an accurate reflection of education on the Professional Development Plan.
- Management
- Plan, implement, manage and evaluate all consumer care.
- Participate in Annual Professional Development Plan and appraisal.
- Provide strong leadership and maintain ongoing communication and education.
- Maintains and improves individual level of IT literacy.
- Contributes to the provision of a safe work environment by adhering to the principles of the Occupation Health and Safety Act.
- Participates in the development and contribution the Community Nursing Risk Register
- Ensures vehicle safety and utilisation is optimised as per the vehicle policy.
- Delivery of consumer and carer centred care.
- Complete annual performance appraisals, forward to Community Nursing Manager for appropriate management through Human Resources in the required timeframe.
- Accurate and timely Data entry reflecting client activity.
- Demonstrates safe and appropriate work practices and identification and appropriate management of potential risks. Able to use Riskman effectively.
- Timely and supportive response to requests by the Clinical Coordinator or Community Nursing Manager.
- Vehicles are clean and safe for use.
- Maintains a tidy and safe work area at all times including the safe management of client information.
- Continuous Quality Improvement
- Program management applying Plan, Do, Check, Act (PDCA) for evaluating practice guidelines and processes, and consumer satisfaction.
- Access PROMPT for all CAH Policy and Procedure Documentation, and participant in the development and review of all new and revised policies as requested.
- Actively participate in all Quality improvement activities, as requested by Community Nursing Manager including stock rotation and supply.

- Registered Nurse holding current registration with the Health Practitioner Regulation Agency, with demonstrated experience as a team leader

- Knowledge and understanding of government legislation, policies and procedures risk management and OH&S relative to Community Nursing within the Public sector.
- Demonstrated ability to contribute to the development of policy and standards and operational procedures within the Community Nursing sector.
- Current Victorian’s Drivers Licence
