Job Title:Flexi-bank Facilitator for Clinical Education

Reports To:Head of Education and Development/Hospice Clinical Educator

Accountable to:Patient Services Director

Responsible for:Supporting theHead of Education to deliver palliative and end of life care training to internal colleagues, volunteers and a variety of external Health Care Professionals
The post holder will link clinical training, education, research and clinical practice, to enhance the quality of palliative and end of life care to all patients, their families and carers.
Main responsibilities:

1. Clinical Practice

1.1Working at an enhanced practice level under the direction of the Head of Education and Development in the provision of leadership in education, training and professional development of health and social care providers (HCP) and volunteers in the community.

1.2Have an in-depth understanding and awareness of the behaviours and interpersonal skills that are vital to an effective learning experience.

1.3Play a supportive role in the training and support of hospice staff, patient care volunteers and visiting students

1.4Play a supportive role in the implementation and delivery of bespoke training programmes for external providers of palliative and end of life care.

1.5In conjunction with the Head of Education and Development, ensure that end of life care tools such as the Gold Standards Frameworkand Advance Care Planning are used effectively to provide a seamless service to patients.

1.6Provide clear explanations and education as appropriate to ensure that patients and families have the information they require in a timely fashion.

1.7Identify an area of special interest to study and act as a specialist resource for colleagues, both internal and external.

1.8Maximise the use of the Hospice Learning Management System Database to support the hospice clinical teams and provide training evidence as required for legislation.

1.9Assist with the development of all HCPs to enable them to provide advice and

guidance related to symptom management, psychosocial and spiritual needs,referring appropriately to all relevant services or services.

1.10Ensure professionals have appropriate links to colleagues in the organisationensuring continuity of advice and intervention through effective team

1.11 Challenge professional and organisational boundaries, identifying areas of skills, knowledge and development to provide continuity of safe and effective high quality patient centred healthcare.

1.12In the best interests of patients and families, proactively collaborate with a wide range of community and hospital based health and social care services including voluntary support groups.

1.13 Promote the work of the organisation in all settings, encouraging collaborative working and the referral of every appropriate patient.

2. Administrative and Managerial

2.1 Provide a point of expertise and knowledge for education and training issues throughout the organization.

2.2Develop effective working relationships with all members of the multidisciplinary team both internal and external.

2.3 Actively participate in service development initiatives as appropriate.

3. Education, Training and Professional Development

3.1Maintain own expert clinical knowledge and skills, including advanced communication skills, and achieve the educational standards set out in line with the skills, knowledge and competency level required of enhanced practitioners and by the role.

3.2Demonstrate advanced communication skills by initiating discussions of legal, ethical and professional issues related to palliative and end of life care, whilst empowering staff, volunteers and external healthcare providers.

3.3Maintain effective links with educational and palliative care organizations as appropriate.

3.4To participate in the Hospice multi-disciplinary internal and external teaching

programme, and undertake formal and informal Hospice teaching programmes,

including students undergoing courses or gaining experience in the Hospice.

3.5 To promote a collegiate, innovative, creative and supportive learning environment

that welcomes change and development.

4. Quality, audit and research

4.1 To participate in the development, review and audit of quality standards and

contribute to the process of continuous quality improvement throughout the

Hospice.

4.2 Keep abreast of research and audit activity to ensure that his/her practice is appropriately updated in line with latest guidance, research findings and organisational policies and procedures.

4.3 Assess report and manage risks in line with the Nursing and Midwifery Council’s Code and the organisation’s policies.

4.4 To comply with all the Hospice’s policies and procedures.

4.5 Contribute to the Clinical Governance framework of the organisation, and keep up to date with governance activity and latest policies and procedures.

4.6 Participate in research and audit aimed at enhancing patient or family care.

4.7 Abide by all organisational policies and guidance in relation to confidentiality and

information governance. Any disclosure of confidential information (including personal information kept on computer or other media) made unlawfully outside the proper course of duty will be treated as a serious disciplinary offence.

5. General

5.1 Organise and prioritise any competing demands of the role, completing duties as required within working hours.

5.2 To participate in the performance review and appraisal process and undertake

performance review and appraisal for team members as designated by the manager. Attend all statutory and mandatory training alongside other training and development opportunitiesidentified through individual performance reviews.

5.3 Adapt and develop in line with changing needs of the role.

5.4 Act as an ambassador for Heart of Kent Hospice in order to raise the profile of the organisation at a local, regional and national level as required.

5.5 Work flexibly across departments from time to time as may be requested by their managers.

5.6 Undertake other duties commensurate with the seniority of the post as maybe requested by their managers.

The post holder will work within the NMC Code of Conduct and Scope of Professional Practice at all times. All organisational policies and procedures will be followed.

This job description is a broad reflection of the current duties. It may be amended and developed in the light of experience and the needs of the service, and will be the focus for objective setting in an annual appraisal.
