Introduction

The JAC appreciate your support but completion of the form is voluntary and the majority of questions have the option of “I prefer not to answer this question”.
The information you are asked to give on the Application Monitoring Form plays no part in the selection process.
The JAC use the information collected to analyse anonymised data, in order to:
• Uncover unfairness or discrimination/disadvantage;
• Highlight possible inequality; and
• Explore the underlying causes.
