THE VETTING CHECKS APPLICATION FORM

The Vetting Checks Application Form is for completion by individuals who are currently serving or have served in the British Armed Forces and their partners when undergoing safeguarding checks with the aim of working with children and/or vulnerable adults. Please refer to Safeguarding Vulnerable Groups Act 2006 for definitions of children and vulnerable adults.

We aim to complete vetting checks within 6 – 8 weeks of receipt of a correctly completed form. Please note forms containing errors will be returned for amendment which will delay the process.

SECTION A – Personal Details

Please provide any other names you may have used and the corresponding dates.

SECTION B – Position Applied For

In order to carry out checks effectively we need to know what work you are applying to do. Please be aware that we are only able to carry out checks on applicants who wish to work with children and/or vulnerable adults.

SECTION C – Service Details

Vetting information held by SSAFA Social Work Service, Army Welfare Service and Naval Personal & Family Service is linked to an individual’s Service Number so we require all the Service Numbers that you have used. You may have more than one because you transferred from (e.g.) Army to Navy, or perhaps you have been commissioned in which case your Service number has changed.

If you are the partner or former partner of a Serving person, please provide their Service number.

SECTION D – Children’s Details

Please provide details for all the children that are in your household.

SECTION E – Address History

Please provide residential addresses (this may be married quarters, single accommodation or a mess) as well as your unit addresses, covering the last five years.

SECTION F – Consent

By signing and dating this section, you are giving your consent for us to carry out checks on behalf of the organisation that has asked you to complete this form.

Data Protection

SSAFA will handle the information contained within your application form, and that which is supplied to you by any third party agencies which we contact, in accordance with the Data Protection Act 1998. Under the Act you have certain rights regarding accessing the information that SSAFA holds on you. You can request to see the personal information that SSAFA holds; in this instance please make a written request to the Data Protection Officer at SSAFA.
