Requirements for a $1,000 Hardin Memorial Hospital Auxiliary Scholarship

Applicant must:

·  Have intentions of entering or have already been accepted in a medical field of study at an accredited college or trade school

·  Applicant must be a current resident of the 10 county service area that HMH

serves or a graduate from a high school in the 10 county service area which

HMH serves (The 10 counties are: Hardin, Larue, Meade, Breckinridge,

Grayson, Nelson, Hart, Bullitt, Green and Taylor)

·  Provide a copy of a high school/college transcript with a minimum 3.0 GPA

·  Be enrolled in at least nine semester college hours

·  Provide a written recommendation from a teacher/instructor

·  Provide a brief summary of why you are requesting this scholarship

·  Apply by May 1st for consideration

Applications will be screened by a committee from the Auxiliary to select the recipients. The HMH Auxiliary Executive committee will approve the selection.

Applications will be available online (hmh.net) or can be picked up at the Director of Volunteer Services or the Gift Shop Managers offices. The application and accompanying documentation should be returned to Hardin Memorial Health, Attn: Director Volunteer Services, 913 North Dixie Ave, Elizabethtown, KY 42701.
