Scope of Work, revision in “Qualifying Organizations” (Page 23):

QUALIFYING ORGANIZATIONS:

DSS will consider awarding funding to organizations that have a proven history of success in providing effective pregnant and parenting teen services. Preference will be given to organizations that are able to provide services to the target population, described below, in rural FresnoCounty. Eligible applicants must be legally established entities and may include, but are not limited to, the following: government agencies, non-profit organizations, private for-profit corporations, and faith-based organizations.

Scope of Work, insert the following under “Qualifying Organizations” (Page 23):

TARGET POPULATION:

Services shall be provided to pregnant or parenting female or male teens residing inFresnoCounty. Clients must be ineligible for Temporary Aid for Needy Families (TANF) or CalWORKs assistance. Preference shall be given to vendors that are able to provide services in rural Fresno County (cities and unincorporated areas) including but not limited to Coalinga, Huron, Firebaugh, Fowler, Kerman, Kingsburg, Mendota, Orange Cove, Parlier, Reedley, Sanger, San Joaquin and Selma. Scope of Work, Section B.23 (Page 23):

Question: Is the awarded vendor required to close cases once the client (male or female) is no longer eligible due to age?

Answer: Yes. Effective July 1, 2010 pregnant or parenting femalesare eligible for services through 19 years of age and males through age 20 years of age. For further details, please see the State’s AFLP Policy and Procedure Manual, Entering and Exiting AFLP (Page 27) which may be accessed by clicking here:

If however, there are extenuating circumstances, the County may petition the State on behalf of the vendor, for a waiver to enroll pregnant and / or parenting adolescents over 19 years of age for females and 20 years of age for males. The waiver may be accessed by clicking here:

The maximum amount of time a client may be enrolled in the AFLP program is 18 months. It is recommended that vendor’s enroll clients that are young enough in age to obtain the benefits in participating in this program (i.e. enrolling a female at 18 and a male at 19).

If it is known that the AFLP client will meet the exit criteria, preparations for referral services and support for the client should begin at least three months prior to the loss of eligibility. The exit process should include:

  1. Development of an exit/transition plan for each client whenever possible;
  2. Initiated at least three (3) months prior to anticipated exit date for clients who will lose program eligibility due to age or who meet the program goals;
  3. Document the reason for exit in the client’s chart, and complete the appropriate State MIS data form

Scope of Work, Section B, insert after 25 (Page 23):

  1. Outreach Activities: Provide outreach activities to schools, community organizations and individuals. Track and document events, home visits, follow up telephone calls and correspondence.

Cost Proposal (Page 26)

The vendor may bill the County a maximum of 158 Months of Service (MOS) per month. These MOS represent clients served per month. For the first six month period, a maximum of 948 MOS will be paid to the vendor.

In addition to filling out Exhibit B, Summary of Cost per Client, vendors may also submit a narrative or description to supplement the submitted budget.

Effective October 2010, documented outreach activities will be eligible for reimbursement on a per client basis and counted as a MOS. The maximum MOS will remain the same.

  1. Preference will be given to bidder’s that provide an in-kind match in addition to resources funded by this proposal.

Award Criteria, insert after Capability and Qualifications, Section I (Page 30):

  1. Preference will be given to bidder’s that include services in one or more for FresnoCounty’s rural areas.
  2. Preference will be given to bidder’s that demonstrate effective collaborations with existing community organizations, churches and / or government agencies.
