REQUEST FOR INACTIVE RECORDS DISPOSAL
Procedure for Records Pick-up Request
· Please copy the attached template onto your school or department letterhead, add the requested information, and send via Pony to the Central Warehouse.
· All documents must be in sealed boxes/cartons and each box/carton numbered (i.e., 1 of 3, 2 of 3, 3 of 3, etc.).
· The number of boxes/cartons must be listed on the Inactive Records Disposal Request letter, as well as a brief description of the box/carton content.
· Principal or Department Head’s signature is required.
· Central Warehouse’s dispatch foreman will schedule a pick-up.
· The boxes/cartons will be sent to the incinerator for disposal or to Records Retention for storage.

This request is for Warehousing Services to pick-up INACTIVE RECORDS from our location. The records are to be disposed of in accordance with the School Board’s policies and procedures; the records are in compliance with School Board’s records retention requirements.
· All records will be sealed in boxes/cartons.
· All boxes/cartons will be labeled “Inactive Records/ Folders.”
· Requesting school/department assumes all responsibility for the box/carton contents.
· Sealed boxes/cartons are to be numbered (i.e., 1 of 3, 2 of 3, 3 of 3, etc.)
