REQUEST FOR EXCEPTION TO REFUND POLICY

A.Purpose:

1.The purpose of this form is to allow the opportunity to request an exception to the refund policy. The form should be completed in detail so that there will be a clear understanding of your request.

B.Process:

a.What are the extenuating circumstances that you think qualify you for an exception to the refund policy?

b.Submit documentation or materials, if any, to support the basis for your request. (Example: Extensive or severe health-related reasons must be accompanied by a doctor's verification.)

c.How can your request be justified under state law and college policy? See the reverse side of this form for more information about RCW 28B.15.605.

A written response to your request will be made within ten business days of receipt of request and supporting documentation.

REFUND POLICY

Students who officially withdraw through the Admissions and Records Office may receive a refund of tuition and fees according to the following schedules:

A.For classes that last at least seven weeks and begin the first week of the quarter:

1.100% refund if a student withdraws prior to the sixth day of the quarter.

2.50% refund if a student withdraws between the sixth and the tenth day of the quarter.

3.40% refund if a student withdraws between the eleventh and the fifteenth day of the quarter.

B.For classes beginning after the first week of the quarter, refunds will be calculated consistent with the above rules.

C.100% refund if the class(es) are cancelled by the college.

D.To receive a refund for non-credit (community education) classes, you must withdraw from the class 48 hours before the first class meeting.

E.Refunds for students receiving financial aid will be made directly to the sponsoring agency (not the student).

F.All refunds will be made by check and mailed. The process takes 10 working days from the date received at the Cashier's Office.

G.Students who believe extenuating circumstances justify an exception to the policy may complete this Refund Exception Form at the Admissions and Records Office.

STATE LAW AND CENTRALIA COLLEGE POLICY

Exceptions to the refund policy are based on WashingtonState law and college policy. RCW 28B.15.605 states specifically that community colleges "may extend the refund...period for students who withdraw for medical reasons or who are called into the military service of the United States." Documentation is required.
