How to use this template:

  1. Include / exclude entry requirements and PPE information as it pertains to your laboratory.

Example: If safety glasses / lab coats are required at all times, display this requirement.

  1. Include a unique name for your laboratory, if you have one.

Example: “Saskatchewan Isotope Laboratory”

  1. Have at least two contact persons listed.

The primary contact should be the person most responsible for laboratory activities (e.g. lab manager).

The emergency contact(s) should be well-informed laboratory members (e.g. senior technician, principle investigator, etc.).

  1. Include / exclude hazard pictograms as it pertains to your laboratory inventory.

Refer to an updated chemical inventory list for relevant hazards in you laboratory, and make sure the sign displays which hazards are present in your laboratory, and which are not.

Delete pictograms for hazards that are not relevant to your laboratory.

Example: A chemical laboratory with no biohazards or nuclear substances should not display biohazard or radioactive hazard symbols.

  1. This door sign does not replace specific biohazard or radioactive hazard signage.

Contact the Biosafety Team for biohazard signage guidelines.

Contact the Radiation Safety Team for radioactive hazard signage guidelines.

  1. Complete the Lab-Specific Information section and post in a conspicuous location for all lab users to see.

If items are kept in a physical location, refer to the location in clear language

Example: First aid kit kept on shelf to the left of main door.

If items are stored digitally, show the file location on this list.

Example: SDSs found in shared drive - W:\Safety\SDS for Lab 123
