FUNCTION:Provide the majority of Clinical Training for all Visiting Staff at UR Medicine Home Care – Hospice, CHHA, LHCSA, Home Infusion). Work with Clinical Operations with on-boarding, precepting, scheduling and other supports.

REPORTS TO:Manager of Clinical Education

RELATIONSHIPS:Administrative Team, Department Managers, PHV staff, Trainers, Support staff, and affiliating agencies.

HOURS:40 Hours, predominately M-F, some evening and weekend hours.

RESPONSIBILITIES:

  1. Provides instruction and the oversight of clinical instruction on documentation processes and evidence –based care practices that meets agency standards for regulatory compliance.
  1. Lead instructor for new clinical employee orientation and on-going clinical education.
  2. Assesses orientee’s experience level to determine level of support and instruction required.
  3. Assesses the orientee’s learning style and plans learning experiences accordingly.
  4. Develops goals and objectives in collaboration with the orientee and the Clinical Team Manager.
  5. Assists the orientee in organizing and prioritizing daily patient care routines.
  6. Reviews the orientee’s documentation
  7. Provides regular feedback to the orientee and evaluates their progress toward fulfilling designated goals and objectives on a weekly basis.
  8. Collaborates with the Clinical Team Manager and Manager of Clinical Education to evaluate progress and address any training issues.
  1. Collaborate with Clinical Operations on the completion of the six-month probationary evaluation
  1. Demonstrate customer responsiveness in education programming and serve as educational consultant and support for activities to influence quality patient care.
  1. Provide oversight and support to clinical instructors and Preceptors as needed.
  1. Demonstrate sophisticated communication skills in interactions with others.
  1. Participate in Agency Quality initiatives as appropriate
  1. Responsible for the Key Performance Indicators (KPI’s) established by the Manager.
  1. Other duties as assigned.

QUALIFICATIONS:

  1. NYS license as a Registered Nurse, Physical Therapist, or Occupational Therapist[AD1]
  2. Minimum of a Bachelor’s degree
  3. Minimum of three years of home health care experience with current, comprehensive knowledge of home care.
  4. Teaching experience preferred.
  5. Experience in clinical supervisory and preceptorship. Ability to mentor professional staff in Case Management.

5. Clinical expert in assessments, interventions, communication, and documentation.

6. Experience with McKesson preferred.

7. Knowledge of Medicare, Medicaid, and other funding source regulations as applied to home care services.

8. Strong experience in OASIS or HIS preferred

9. Excellent interpersonal and communication skills, both oral and written.
