Report Person and Family Engagement (PFE) in SmartSheet
GOAL: Strengthen person and family engagement (PFE) as partners in their care.
As an atom Alliance associate, you should be continually soliciting feedback from stakeholders to inform and enhance PFE work across the region. It is vital that you expand existing avenues for two-way communication existing avenues for two-way communication with beneficiaries. We must report specifics of your good work in this area to our COR on a monthly basis.
Please use this SmartSheet form, which allows the atom Alliance to:
- Document interactions where we meaningfully include patients and advocates (including, but not limited to, members of our patient and family Advisory Council) in our QI initiatives
- Record how we support meaningful, intentional application of PFE principles to our work
- Highlight areas where our QIN-QIO is driving implementation of PFE principles in provider settings
- Identify what the next steps are in moving forward with PFE work
