Report by Placement Supervisors for Anglican Candidates

The Report should be written by the Placement Supervisor after some initial discussion with the student at the end of the placement. The student and supervisor should then talk through the final draft of the Report, which should be signed by both the Supervisor and the student concerned. If the student has already left the placement, making meeting problematic, then the conversation can take place by phone or email and the signing done sequentially by email (and noted as such).

It is understood that the student’s signature does not necessarily imply agreement with all the views expressed by the Supervisor.

The student may take a copy of this form before returning it, for future reference, and the completed report must be sent, either by you or the student, to Sarum College within a fortnight of the end of the placement please.

Notes to Placement Supervisor

Please find following a form which is designed to help you make your contribution to our reporting procedure on candidates in training for Ordination or Licensed Lay Ministry. Although you have only known the student for a brief period, this is a valued report.

This hard copy is provided for reference, but please use the electronic version of the form to complete it which can be found on the Sarum College website at the bottom of the page , looking for Placement Supervisor Report form.

To download from the web, simply open the Microsoft Word version of the form and then use the ‘save as’ function of your computer to save it wherever you want on your local drive.

When you save the file, please save it as: placement report [student surname], e.g. placement report Smith.

  • Each heading asks you to assess a particular aspect of your student’s development, such as you perceive it, over the placement time. Please do not write more than about 50 words under each heading. Do not feel compelled to comment onareas you feel unable to.
  • It may be that the heading itself is enough of a prompt for you. If you want more specific guidance, the points in italics provide examples of specific areas which you might want to talk about. Please write in whole sentences for clarity and to enable us to quote your responses accurately. Please remember that, in the case of Anglican candidates for ordination, this report is for the Bishop.
  • Having completed the form, please check that you have used it to give the best possible indication of how your student fits her/his calling, and how this can best be supported by future training. Please give attention to strengths as well as weaknesses.
  • It is not your role to make a final recommendation about ordination or authorization, but you should raise any substantive issues of this nature on the final page of the form and the college will take these up with your student.
  • As you will notice from the end of the form, your response must be read and discussed with your student before it is returned to Sarum College. You are not required to agree on all the views that you express; it may be necessary to articulate a diversity of perspective. Electronic signatures are permissible, as: name (electronic).
  • Please can you or the student return the completed forms to Annette Young, Sarum Administration Office as an attachment to an email within a fortnight of the end of the placement.
