E4libraries Accreditation Scheme

Renewal of Accreditation form

______

Rather than asking organisations that have already gained an award on the e4Libraries Accreditation Scheme to apply for renewal / re-accreditation from scratch, the following is an update form which can be used to highlight any significant changes or progress made in the last year (or since your organisation’s previous application).

Please note: if you feel that one or more of these areas is not applicable to your organisation, please explain why in the field provided. If you feel that you have already implemented a technology and derived all possible benefits in a previous year's application please answer 3 (fully implemented) rather than 0 (no new progress made).

Note, if you are applying as a library stock supplier rather than a library, please give details of what you do and how you assist your library customers in these areas.
