Relentless Lacrosse Player Contract
Each player and parent must read the policies below. Each player must initial and sign where indicated to acknowledge understanding and acceptance of these policies. The form must be signed by both the player and the parent and returned to the coach at the first practice.

______/ I understand that I must respect the authority of all coaches. I may not speak when a coach is talking to the team, and that I must remain attentive even when I am not in the drill because it delays the entire practice.______/ I understand and accept the importance of practice and game attendance. Any absence that has not been submitted to Brian Eisenberg at the point that the schedule is announced in full is unacceptable. A player that is injured, but will return at any point during the spring / summer, must continue to attend all events. A player who becomes ill should attempt to make all events to watch only, dressed in appropriate weather clothes, unless bed ridden or contagious.
______/ I understand and accept that the coach may not give all players an equal amount of playing time and that decisions about playing time are at the sole discretion of the coaching staff.
______/ I understand and accept that it is my responsibility: (1) To learn and abide by the rules of Lacrosse (2) To ask questions of my coach or other league official when I am not clear on the rules (3) To use ALL mandatory equipment in the proper manner during practices (Mouthguards included) and to notify my coach if my equipment becomes illegal to use during the course of the game.
______/ I understand and accept that Relentless Lacrosse requires an atmosphere of good sportsmanship and it is to be maintained at all practices and games. Poor conduct will not be tolerated including but not limited to:
1. Enter into any argument with an official as to any decision that has been made or in any way attempt to influence the decision of an official. 2. Use threatening, profane, or obscene language or gestures at any time during practices or games. 3. Bait, taunt, call undue attention to oneself or any other act considered unsportsmanlike by the officials.
______/ I understand and accept any player involved in a fight or any other action deemed inappropriate may be removed from the game current game, and suspended from the team's next game. A second violation may result in suspension for 12 months. The is a ZERO tolerance for parents who act inappropriately!
______/ I understand and accept that Relentless Lacrosse prohibits use of illegal drugs and alcohol by players participating in practices and games, or any event associated with the league, including to and from events. If this is violated it will result in immediate suspension from any Relentless activity up to 12 months. A player in the company of another player who is openly violating this rule will also be considered in violation of this rule and subject to expulsion from the team.
______/ I understand that Relentless Lacrosse will be unofficially representing The Headstrong Foundation. All players are required to be able to explain, both in writing and verbally, what the Headstrong Foundation is and participate in the “Score for a cure “ fundraiser. A one page paper summarizing the Headstrong Foundation is due on the first day of practice with this contract.
______/ I understand and agree to participate in all Relentless Lacrosse fundraisers and off field community activities.
______/ I understand that we will be traveling as a team without your parents. I will be respectful and safe at all times while traveling. I will always “partner up” with a teammate and notify an adult before separating from the team at any time (bathroom, food, etc.). I will be responsible for showing up on time for team meetings (In the mornings and when leaving hotel for games, meals, etc.) I will always keep the cell phone number for an adult on the trip with me at all times.
______/ I understand and agree to comply with the terms of this player contract. I agree that if I am removed from the team’s roster for disciplinary reasons, I will not be entitled to any refund of fees paid to Relentless Lacrosse.
