You must have post registration experience andspecialist palliative care / Day Care experience is advantageous. You must also be able to demonstrate highly developed clinical and leadership skills as well as effective working in the context of a multidisciplinary team. A knowledge and commitment to learning and continual improvement is essential. Due to use of electronic patient care records, IT literacy would be an advantage.

2.JOB PURPOSE

  • The delivery of expert, supportive and compassionate palliative care which benefits patients and their families/carers.
  • Assessment and management of holistic needs in bespoke, creative and enabling ways.
  • In absence of Day Care Manager, take responsibility for the daily operations
  • Create a safe care environment which supports and enables patient wellbeing.

  • To provide holistic, specialist palliative care to people living with a life limiting illness and palliative care needs
  • Through a rehabilitative approach to care, to enable patients to remain at home and live full and supported lives for as long as possible.

  1. DIMENSIONS
  • The Day Hospice provides access to specialist palliative care for people within Forth Valley and North Lanarkshire to find support including the need for pain and symptom management, social, spiritual and emotional support. The unit has capacity for 20-25 patients per day Monday to Friday with a total caseload of 100-120.
  • The post is a clinically focused job with some requirement for supervising and teaching roles.

6.KEY DUTIES/RESPONSIBILITIES

Clinical Practice & Leadership

  • To assess and manage a range of symptoms in collaboration with patient and multi-disciplinary team
  • Build and maintain therapeutic relationships with patients and, where appropriate, their families, to address ongoing and developing social, psychological and spiritual needs.
  • Negotiate clinical options and decisions with the patient, family and multi-professional team, using a rehabilitative approach where appropriate;
  • Work with patients effectively to optimise their knowledge of and engagement with treatments (pharmacological and non-pharmacological) to enhance their quality of life.
  • Identify hopes and goals of the patient and work with them towards achieving these;
  • Anticipate the changing needs of patients and explore their awareness and emotional response to their situation.
  • Explore the support needs of carers, and maximise opportunities to respond to these
  • Be competent in managing a range of interventions to meet the full range of patient needs.
  • Support active, safe, timely discharge of patients from the service, ensuring that it is planned and executed in collaboration with individuals and their families and the multiprofessional team.

Communication

  • Build healthy relationships with a range of patients in order to support them in adapting to living optimally with their illness.
  • Keep carersinformed of all key decisions;
  • Communicate with patients and others demonstrating the use of appropriate language, summarising, reflecting back, clarifying and challenging;
  • Demonstrate effective communication skills, particularly around the management of fear, anger, collusion, denial;
  • Proactively engage with patient education activities (individual or groups).
  • Work in partnership with other professionals;
  • Demonstrate respect for skills and competencies of the multi-professional team;
  • Demonstrate concise, non-judgemental written skills.

Education & Training

  • Utilise both formal and informal methods to teach nursing/other staff;
  • Contribute to a learning environment, enabling others to develop and apply their knowledge and skills in practice

Grief, Loss & Bereavement

  • Assess coping strategies of individuals and their families with regard totheir own death, dying and bereavement.
  • Support patients who are experiencing loss of many kinds
  • Encourage a pro-active approach to creating memories
  • Support parents by giving practical advice regarding children facing loss;

Management

  • Co-ordinate the department as required to ensure continuity, multi-professional action plans, up-to-date assessment of patient need and staff expertise;
  • Use time effectively, according to priority and need;
  • Ensure safe care by assessing and managing risk within the environment and for the journeys to/from home
  • Act appropriately in the event of a fire or serious incident;
  • Take responsibility for building good team relationships / managing conflict;
  • Report poor performance to a relevant person for appropriate action to be taken;
  • Demonstrate an awareness of the organisation as a whole including the needs of other departments/services.
  • Ensure adequate supplies are available to deliver a safe and seamless service.

Quality

  • Contribute to an ethos of continual quality improvement, utilising adverse incidents and feedback to learn and develop practice.
  • Work with Hospice quality systems to evidence sound and consistent best practice.

Professional Development

  • Participate in the Strathcarron professional review and development system.
  • Work in a manner which reflects the NMC Code of Conduct and the standard and policies of Strathcarron Hospice.
  • Maintain valid registration with the NMC for the duration of employment
  • Attend agreed courses and study days in order to develop knowledge and skills.
  • Take responsibility in all areas of work for safeguarding people at risk (both adults and children), from any form of abuse or neglect
  • To be a mentor to qualified and unqualified nursing staff.
  • To act as required as a key worker for special interests i.e. moving and handling; infection control; lymphoedema – incorporating the skills of teaching, role modelling and expert practice in the specific field.
  • To participate in audit and research initiatives and promote research based nursing practice including standard setting.
  • To ensure ongoing professional/personal development.

7.SYSTEMS AND EQUIPMENT

Systems:

  • IT systems

Equipment:

  • Moving and handling equipment e.g. Hoists, patient slides, baths, pressure relieving mattresses, profile beds, wheelchairs, walking frames.
  • Respiratory equipment e.g. nebulisers, suction, oxygen and humidification delivery systems.
  • Infusion devices for subcutaneous,Intravenousand Intrathecal drug delivery
  • Automated external defibrillator.
  • Patient monitoring equipment e.g. Dynamap, Tympanic thermometers, Blood Glucose monitoring.
  • Feeding equipment for use with enteral support eg PEG feeding.
  • Therapeutic/ Communication Aids
  • Be competent in the use, storage and maintenance of all equipment used within the clinical area of work.

8.ASSIGNMENT AND REVIEW OF WORK

  • The staff nurse will be responsible to the Day CareManager for clinical guidance and professional management, work review and formal appraisal of performance.
  • The post holder will be expected to take charge of the department on a regular basis.
  • The post holder will have responsibility for managing a defined workload within professional guidelines. Workload will be prioritised on a daily basis.
  • The Staff Nurse will delegate/allocate work to the team in the absence of senior staff. Workload will be prioritised on a daily basis.

9.COMMUNICATIONS AND WORKING RELATIONSHIPS

The post holder will be expected to:

  • Manage the group dynamic within the Daycare environment
  • Communicate with all members of the multidisciplinary team.
  • Communicate patient care needs and work allocation to junior nursing staff.
  • Communicate with and oversee the work of volunteers in the department.
  • Communicate with other departments within and external to the Hospice
  • Be aware of patient referral system to other professionals, internal and external to the organisation.
  • Maintain verbal and written communication systems within the Unit

and with other departments.

  • Maintain a professional manner when dealing with patients, staff and relatives, ensuring patient confidentiality.
  • Be aware of the diversity of culture, religion/individual communication needs.

9a. PHYSICAL DEMANDS OF THE JOB

  • Stand/walk for the majority of the shift.
  • Moving patients, some who have a high level of dependency, complex needs and associated risk and ensuring Hospice moving and handling policies are adhered to.
  • Moving of furniture/equipment as required.
  • Be able to respond speedily to emergency or unplanned situations.
  • Managing / de-escalating physically aggressive patients.
  • Exposure to body fluid on a daily basis and unpleasant smells.

9b. MENTAL/EMOTIONAL DEMANDS OF THE JOB

  • Supporting patients through their episode of care.
  • Breaking bad news to patients and relatives.
  • Supporting patients and relatives who receive bad news.
  • Acting as patients advocate when appropriate.
  • On a daily basis, care for people in advancing illness particularly those in the younger age group.
  • Providing pre-bereavement support to patients and their carers.
  • Giving telephone advice to patients/families with concerns using clinical judgement as required to advisethe patient appropriately.
  • Being prepared at all times to deal with verbal complaints or aggressive behaviour from patients and/or relatives.
  • Unpredictable workload.
  • Concentration required for majority of shift
  • Involvement with ethical and moral dilemmas on regular basis.

10. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB

  • First Level Registered Nurse.
  • Degree level study in relevant subject preferred.
  • Willingness to undertake post-graduate study.
  • Experience of team working with ability to work using own initiative.
  • Highly developed skills in managing sensitive communication with patients, families and teams.
  • Mentorship qualification
  • Experience of supporting, managing and teaching junior staff
  • Time management skills.
  • Be familiar with Hospice policies.

Terms and Conditions:The terms and conditions of service are those laid down in line with the Whitley Council terms and conditions.

Salary Scale:Band 5, £22,218 to £28,746 per annum pro rata

Hours:16.5 hours per week

Employees may be required to vary the pattern of their working week subject to the exigencies of the service.

Duration:Permanent.

Annual Leave:27 days rising to 29 days after 5 years’ service and to 33 days after 10 years’ service plus 8 public holidays (pro rata).

Or

202.5 hours rising to 217.5 hours after 5 years’ service and to 247.5 hours after 10 years’ service plus 60 hours public holiday (pro-rata)

Superannuation:This post is superannuable under The Falkirk Council Local Government Superannuation Scheme and your remuneration will be subject to deduction unless you choose to opt out of the scheme.
