The delivery of high-quality, patient-centered health care requires integration from many parts of the continuum, including effective coordination of transitions between providers and care settings. Poor coordination of care across settings may result in readmissions, many of which can be prevented. Working to reduce readmissions is one way of addressing quality improvement in your rehabilitation facility.
This half-day workshop examines the problem of readmissions back to acute care and readmissions after completion of the rehabilitation program within 30 days. The workshop will describe the current research and identify potential risk factors in a variety of conditions that place these patients at high risk for re-hospitalization. The workshop will also highlight pre-admission considerations and discuss actionable risk-reduction strategies that can have a positive effect on rehabilitation healthcare.

Uniform Data System for Medical Rehabilitation

Pam Roberts, PhD, is manager of rehabilitation and neuropsychology at Cedars-Sinai Medical Center in Los Angeles, California. She also teaches at the University of Southern California and guest lectures at other local universities. Dr. Roberts is an international FIM® trainer for Uniform Data System for Medical Rehabilitation in Buffalo, New York, and is a surveyor for CARF: The Commission on Accreditation for Rehabilitation Facilities. She has worked throughout the continuum of care as both a clinician and an administrator.
Dr. Roberts currently serves on the board of the California Hospital Association (CHA) Center for Medical Rehabilitation Services. Through her role in CHA, she has been actively involved in the Recovery Audit Contractor issues in California. She has led strategic planning sessions regarding RAC issues and has worked with CHA to develop an approach for dealing with the issues with the contractor. She has also worked closely with the Centers for Medicare and Medicaid Services on these issues.
Dr. Roberts has presented a variety of workshops and given numerous consultations regionally, nationally, and internationally. In 2007, she was awarded her PhD in health sciences, studying the effect of comorbidities on functional outcomes.
