Rays Community Fund Grant Application

The Rays Baseball Foundation is committed to supporting youth and education programs throughout the Tampa Bay region. The Rays Community Fund Grant Program provides assistance to local non-profits in the Tampa Bay region. Grants up to $5,000 are awarded through this program. These funds should help add to and support current programs offered by community-based non-profit organizations.

This application contains four (4) sections. These sections will ask for: General Information; Financial Information; Organization Information; and a Grant Proposal.

Grants will be awarded through two cycles in 2011. Organizations may receive a maximum of one grant award per calendar year; if a grant is not awarded in the first grant cycle, an organization may submit an application again in the second cycle. The deadline for submitting an application for the first grant cycle is: March 1, 2011. Application must be typewritten. Two (2) copies should be submitted via mail to:

Grant Proposal

Please write a 2-3 page proposal on the proposed use of grant funds. The grant proposal should be kept separate from the rest of the information listed above. Please paperclip the pages of the proposal together; do not staple or bind the proposal.

The following information should be included in the proposal:

·  Organization name and name of program to be funded as a header on each page of the proposal

·  Grant amount requested

·  Estimated budget for program

·  Other financial supporters of the program

·  Population of children the program will impact

·  How the program’s goals will be evaluated

·  How the partnership with the Rays Baseball Foundation will be recognized
