Senior Year Field Practice Evaluation – SPRING SEMESTER*

This evaluation presents an opportunity for the student and field instructor to evaluate the student’s growth and development over the full senior year, along ten competency areas developed by the Council on Social Work Education. The standard by which an intern is to be assessed is that of a new entry-level BSW social worker. Please sign and return to faculty liaison/advisor when completed.
