To be eligible for this program please complete the Windstream Enterprise Sales SPIF Form below. Once the form is completed and returned to MACH Networks you will be eligible to receive your SPIF. Payment will be issued directly to the WindstreamEnterprise Agent from MACH Networks within 90-days of the activation date. Please note that you can list multiple devices for a single customer but each customer requires a separate form.

Instructions are as follows:

  1. Complete this form in its entirety. Incomplete forms will not be processed. If more room is needed, please attach multiple copies of page 2 of the Agent SPIF Form (see below) to be able to accommodate a complete list of MDNs (Mobile Device Numbers or cellular phone numbers) and IMEIs (Serial Numbers) of the activated devices.
  2. Be sure to sign and date the form at the bottom of the page.
  3. Once completed, save the file as AGENTNAME-MM-DD-YY (example: MACH-05-11-18) and email the form to .

Terms & Conditions
This offer is valid on new WDB or SD-WAN 4G LTE lines of service that are ordered by Windstream Enterprise and activated by MACH Networks between February 1, 2018 - December 31, 2018. Valid SPIF claims must be submitted within 45 days of the activation date but no later than January 31, 2019. Demo units do NOT qualify for rebate. Offer valid for one SPIF per router per line of service. Offer is only valid when an AT&T 4G LTE line of service is provided by WindstreamEnterprise and may not be combined with other offers. Incomplete claims or claims that do not include the required information shall be deemed invalid. MACH Networks is not responsible for lost, late, mutilated, postage due mail or misdirected mail or emails that have not been received. Offer is valid only in the US and is void where prohibited or restricted by law. Please allow up to 90-days for processing. Rebate checks not cashed within 90 days of issuance are void and will not be honored or re-issued. If service is terminated for any reason within one year of the activation date SPIFs may be reclaimed by MACH Networks or offset against SPIFs on future orders. Retain a copy of all submitted materials for your records. All documents become the property of MACH Networks and are non-returnable. Incomplete forms will not be processed.
