Checklist for QUEST (May 13-26, 2001)

In order to enroll in QUEST the following items must be completed:

 Possess a valid SCUBA certification from a recognized training organization (e.g., NAUI, PADI).

 Possess medical insurance that is valid in the state of Hawaii during QUEST.

 Pass Hawaiian marine organism identification exam at local MOP office with a grade of 80% or better (references and species lists enclosed).

Submit a completed application form and deposit of $200 by February 15, 2001. You may elect to pay the entire balance at this time.

 Upon notification of acceptance into the course (by March 1) the following must be completed:

1. Pay the remaining balance due March 31, 2001.
Resident Tuition QUEST 264/364 = $378 + $2 student fee
Non Resident Tuition QUEST 264/364 = 390 + $2 student fee
QUEST 264 Lab Fee = $450, QUEST 364 Lab fee = $225

2. Obtain a complete physical exam as described in the University of Hawaii Research Diving authorization form and submit to the DSO.

3. Obtain CPR/1st Aid certification valid through June 1, 2001.

4. Obtain a dive check-out by the University of Hawaii Dive Safety Officer

Highly recommended but not required:

Diver Alert Network Insurance (brochure enclosed);

Emergency oxygen use course (contact the UH Diving Safety Officer for details);

Rescue Diver training for MARE 364 students (contact the UH Diving Safety Officer for details).

NOTE: Students will be prohibited from taking QUEST unless all items
are completed before the start of the course. There are NO exceptions!

Please enclose a short essay (one-page maximum) which discusses your academic and career goals and how QUEST will help you to attain these goals. In your essay, briefly summarize your diving experience and your experience with personal computers (during QUEST students will use IBM PC based statistical, word processing, and spreadsheet programs). If you are applying for Advanced QUEST, discuss why you are interested in being a team leader, and describe any background experiences that will help you to function as an effective dive leader.
