PUPPY ADOPTION AGREEMENT
The following sale and agreement between:

And
To be effective as of:
Amy and Rick Krumenauer, DECORAH GOLDENS has this date delivered to the Buyer a boyGolden Retriever
The AKC litter registration number of ______
Sire: GCH Forever All Jacked Up, CGC “Whiskey”
Dam:GCH Forever Decorah Beating the Boys, CGC “Zenn”
The consideration for this sale shall be the sum of $1500.00plus the terms and conditions hereinafter set forth.
ADDITIONAL TERMS:
This puppy has been presented to a veterinarian by the seller and found to be in good health.
That your licensed veterinarian should examine this puppy within seventy-two hours of receipt (if puppy is shipped). If your veterinarian determines the puppy to be ill, or if for ANY reason the puppy is unacceptable, Buyer may return the puppy for a full refund, providing it is in the same condition as at the time of purchase. If puppy is returned due to illness, a letter from your veterinarian is required.
That puppy will be given adequate and proper shade and housing and properly and sufficiently fed and nurtured. This dog shall not be neglected in any manner and in no manner mistreated. Dog will receive adequate and proper veterinary care throughout his lifetime and all inoculations maintained to ensure preventive healthcare.
The buyer agrees to prefix DECORAH at the beginning of the dog’s registered name.
The buyer agrees to promptly notify the seller of any address change made by the buyer.
The buyer agrees to notify seller of any titles completed on dog as determined by the AKC, GRCA, NAHRA as well as UKC and CKC as well as other recognized registry.
Dog nor it’s progeny shall not be used for purposes of vivisection, nor will they be sold by or through any commercial or wholesale establishment, any chain store, catalog sales house, pet store or “puppy mill.”

Puppy is sold with ______Full AKC Registration (Decorah Goldens will be listed as co-owners) That in order for the seller to maintain a quality-breeding program, Seller may request clearances be obtained and will be required of all dogs used for any breeding purposes. Clearances will be obtained prior to breeding and not prior to the dogs 2nd birthday.
That Buyer guarantees Amy and Rick Krumenauer, Decorah Goldens right of first refusal if the dog is to be sold or ownership changes in any way. If the sellers do not wish to acquire the dog, seller by assist the buyer in placing him
This puppy will be replaced should a qualified veterinarian diagnose it in writing, before it is twenty-four (24) months old as having any of the following:
A)Hip or Elbow displasia, diagnosed by the Orthopedic Foundation for Animals below a FAIR reading.
B)Inherited eye diseases as diagnosed by a board certified veterinary ophthalmologist.
C)For puppies sold with a Full Registration any disqualification as outlined in the Golden Retriever breed standard.
This guarantee applies only to the original buyer.
This guarantee null and void if any dog has not been examined prior to 24 months old, or if the dog or bitch has been bred.
This guarantee null and void if dog was not properly cared for; including not being given proper nutrition, exercise or was not within an appropriate weight range based on age.
Seller warrants and agrees that he/she is the lawful owner of the dog, and that the dog is free of all encumbrances, that he/she has the right to sell the same and that he/she will warrant and defend the title to the dog from all lawful claims and demands.
The Buyer has read and understands the foregoing and agrees that all parts and portions will constitute a binding agreement.
In addition, buyer agrees to become educated in matters related to the health and well being of the Golden Retriever and to make educated decisions regarding this puppies training, health and medical care. Included but not limited to:
American Kennel Club (AKC)
Golden Retriever Club of America (GRCA)
Morris Animal Foundation
American Veterinary Medical Association (AVMA0
This contract constitutes the entire agreement of the parties involved in the sale of the puppy.
